Assisted Reproduction
All services provided are limited to $50,000 per covered person per lifetime . Limited to covered persons who are at least 26 years old.
• Limited to 4 or less attempts at artificial reproduction per person per Lifetime
• Limited to the Covered Person who has not undergone more than 4 attempts at assisted reproduction. If a live birth follows 1 of the attempts of assisted reproduction, then 2 additional attempts will be permitted. • Limited to procedures performed at medical facilities that conform to the American College of Obstetrics and Gynecology guidelines for in-vitro fertilization clinics, and to the American Fertility Society minimal standards of in-vitro fertilization programs.

• Limited to services or supplies of assisted reproduction including in vitro fertilization, uterine embryo lavage, embryo transfer, artificial
insemination, gamete intrafallopian tube transfer, zygote intrafallopian tube transfer, and low tubal ovum transfer.
Benefit does not apply to services rendered to a surrogate mother for purposes of child birth.
Standalone — Cryo-preservation and storage of sperm, eggs and embryos are limited to a maximum payment of $2,500 per Calendar Year.
