Introduction: Sexual health centers are often considered to be the appropriate place to initiate Pre-Exposure Prophylaxis (PreP) counseling and increase PrEP awareness. However, previous studies report that among those referred for PrEP, the proportion that attend an intake PrEP appointment varies widely. In many cases, the majority of those referred did not ultimately receive a prescription. Same-day or “Rapid PrEP” (RP) initiation can play a key role in facilitating access to PrEP by consolidating initial PrEP evaluation and initiation to the same day patients present for other services. If evaluation and labs indicate that PrEP is safe and appropriate, patients may take their first dose on the same day as their initial clinic visit. Implementing protocols for same-day PrEP prescribing has been identified by the CDC and others as a key avenue for increasing PrEP uptake. Same-day PrEP initiation protocols have been successfully implemented in clinics in Denver, CO, St. Louis, MO, Chicago, IL, Jackson, MI, and New York, NY. While existing RP programs demonstrate that RP is a highly safe and sustainable way to prescribe

PrEP, challenges remain to improve retention to follow-up appointments after RP initiation.

Discussion: Crucially, no studies to date have systematically asked PrEP candidates how best to adapt a same-day PrEP service to meet their needs. Through focus groups discussions (FGDs) with SHC clients, this study gathered client feedback, concerns, and suggestions on the plan to implement a Rapid PrEP Initiative (RPI) at the Louisiana State University-Crescent Care Sexual Health Center (SHC) in New Orleans, Louisiana, an epicenter of HIV in the South. The SHC sees over 3,000 clients for STI screening per year and is situated in an urban area with some of the highest rates of people living with HIV, accounting for over 4% in some zip codes, in Louisiana, which consistently ranks among states with the highest HIV and STI rates. The objective of this study was to determine client-directed priorities for adapting the plan to adapt the RPI to the local setting based on qualitative analysis of focus group discussions with a range of candidates for PrEP.
