L’Équipe de santé familiale Plantagenet est à la recherche d’un (e) diététiste pour travailler au sein d'une équipe multidisciplinaire de soins de santé afin de fournir des conseils diététiques dans le cadre d'une approche globale de la gestion des maladies chroniques pour la population de patients de sept (7) médecins de famille. The Plantagenet Family Health Team, is looking for a dietitian to work as part of a multidisciplinary health care team to provide dietary counselling as part of a comprehensive approach of chronic disease management for the patient population of seven(7) Family Physicians. Conditions d’emploi/Employment conditions: 3 jours semaine/ 3 days/week Date prévue de début/Start date : Dès que possible/As soon as possible COMPÉTENCES REQUISES/REQUIRED EXPERIENCE Études/Education : • Diplôme universitaire de premier cycle de quatre ans dans un programme universitaire agréé en alimentation et nutrition ou équivalent. • Être inscrit auprès de l'Ordre des diététistes de l'Ontario. • Détenir une assurance responsabilité professionnelle • Completion of a four-year undergraduate university degree from an accredited university program in foods and nutrition or equivalent. • Registration with the College of Dietitians of Ontario. • Professional liability insurance. Expérience/Experience : • Trois à cinq ans de conseil clinique et de nutrition communautaire. encourager, à soutenir et à motiver les patients et/ou leur famille. • Capable de travailler efficacement dans une équipe interdisciplinaire • Three to five years clinical counselling and community nutrition. • Expérience en développement, mise en œuvre, suivi et évaluation de programmes. • Excellentes aptitudes à la communication écrite et orale, y compris une capacité à • Experience in program development, implementation, monitoring and evaluation. • Demonstrate excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families. • Able to work effectively in an interdisciplinary team Langues/Languages: Bilingual Sûreté et sécurité/Security and Safety : Vérification de dossier avec secteur vulnérable/ Criminal record check with vulnerable sector Avantages sociaux après la période de probation.
