WELCOME TO OUR PATHWAY TO PLANETREE

Year in Review

Happy New Year! We want to share some exciting projects and events with you that we hope will raise your spirits and renew your energy! Thanks to our Planetree Steering Committee and key partners from Quality, HR, SW, BH, Medicine, Public Relations and Marketing, and our PFPC, we have identified opportunities that look closely at ways towards achieving the highest standards of safety and quality. The work is built from information gathered by 3 core elements- Staff, Patients, and Community, all of which we will use to illustrate our journey. Our application submission for designation will take place in January 2023 and an on-site assessment will commence in Spring 2023. A Planetree designation is simply an acknowledgment of all the great things we do at Jacobi that makes us proud to be who we are. Here are highlights of some of the projects we have launched in partnership in 2022 with

our collaborative partners: CARING FOR OUR PATIENTS

Jacobi's whiteboard, now named the "Communication Board" project was co-designed by both patient council members and staff and will help build a 2-way communication standard between patients and staff. Patients share their desired goals and personal preferences which will help staff interact with them as individuals rather than by an identity common to HCWs (healthcare workers) such as their “medical condition” (i.e. diabetic patient in Bed 1) Staff provides important info that will help patients understand their care plan more clearly. This will help put our patients at ease so that they are able to relax more and focus on their healing. Proud winners of the Systemwide unit challenge in 2022 for increasing our nurse communication scores by 65%. Congratulations to 5D for their compassionate care and commitment to our patients!

CARING FOR OUR STAFF

Person-centered Leadership training- Middle managers often find themselves “sandwiched” between carrying out directives from their leaders and ensuring staff needs are met so that they can do their jobs. The sessions offered tools that improve communication, build relationships and work toward a resolution. Facilitator training for Connecting to Purpose- Staff will soon provide training for all staff on building communication with empathy and compassion, which will give staff tools to interact with patients, families, and colleagues in a more meaningful way. Caring Culture Survey for Staff- How do we feel about the way we care for each other and our patients, and how we are cared for by others? Results from this survey will take a look at how far we have come since our 2021 survey and what we need to do to stay on track.
