SOS was founded in January 20th, 2000 with the following mission “to break the cycle of substance abuse within our community and enhance the quality of life of the individuals we serve and their families through the alleviation of human suffering caused by alcoholism, drug addiction and other co-occurring behavioral health problems”. SOS is a non-profit state certified (DUI, Drug Testing, Outpatient and Intensive Outpatient) Substance Abuse Agency, whose sole purpose is to help individuals suffering from substance abuse and related behavioral health problems. SOS offers adult and adolescent programs on a sliding scale payment plan; which is meant to ease the financial burdens of each client.

SOS offers the following ODMHSAS (State) certified services: • Early Intervention Programs- Substance Abuse Education Series for adolescents, adults and families to educate on the issues surrounding substance abuse and create change in thinking and behavior. • Outpatient and Intensive Outpatient (IOP) Substance Abuse Programs- For adolescents, adults and families. Services are gender specific and tailored to meet each client’s specific needs. • Women’s Sober House (Outpatient or IOP Services) – Structured sober living environment for women. Outpatient and IOP services are gender specific and tailored to meet each client’s specific needs. • Comprehensive Family Court Substance Abuse Evaluations- Standard Assessment + Third Party Interview • DUI/Sub. Abuse Evaluations- DUI, DOT, Court Ordered and private pay substance abuse evaluations. • DUI Classes/Services- State Certified 24-hr., and 10-hr. DUI schools, VIP, and DUI outpatient services • Impaired Professionals Programs- Providing specific treatment to the impaired professional (nurses, allied health, lawyers, physicians, etc.) with substance abuse and co-occurring issues. • Opiate Addiction Programs- SOS’s medication assistance IOP program. Clients will participate in services monitored by both physicians and clinical staff. • Anger Management/Coping Skills Programs- Focusing on clients with co-existing substance abuse and anger/emotional management problems. • Co-Occurring Substance Abuse Programs- For adults and adolescents suffering from both substance abuse and mental health disorders. • Aftercare/Relapse Prevention Program- Preventing relapse for adolescents, adults and families. • Impulse Control (Criminal Thinking Program) – Cognitive Behavioral Program specifically developed for drug, alcohol, and other criminal charges. Drug/Alcohol Testing Programs: • Laboratory Based Urinalysis (8-panel and 19-panel test options) • Hair Follicle (6 and 10 panel tests, detects regular drug and alcohol use for up 90-days) • EtG Urine Alcohol Test (detects alcohol use 3-4 days after use) • Drug Testing Kits (in home or in office results in 5 min.) • Our staff consists of qualified professionals who have a combined experience of over 35 years serving our community.

Specialized Outpatient Services, Inc. offers immediate result kits with alcohol and drugs and laboratory urine, and hair follicle drug testing. All testing procedures are observed by SOS staff (except under special circumstances). Testing services are offered to private individuals, parents, family members, schools, employers, DHS, probation and parole, attorneys, court systems,

private practitioners, sober living houses, doctors and treatment providers.

EMPLOYMENT URINALYSIS TESTING: SOS offers employers pre-employment, random and reasonable suspicion drug/alcohol testing options that comply with current OESC workplace drug testing laws. URINALYSIS TESTING SOS offers and can test for 1 to 10 panel drug screens (including alcohol). SOS offers a basic urinalysis testing package which consists of • 1 Panel Test: (Cocaine, Amphetamines, Methamphetamine, Marijuana, Opiates, Barbiturates, or Benzodiazepines). • 8 Panel Test: (Cocaine, Amphetamines, Methamphetamine, Marijuana, Opiates, Barbiturates, Benzodiazepines and Alcohol). • Adulteration and Confirmation Testing: SOS is now able to offer adulterant tests on suspicious sample. This includes pH levels, creatinine, specific gravity, and oxidant testing. SOS also offers LC/MS and GC/MS confirmations to verify results (gold standard of testing). ETG- URINE ALCOHOL TESTING The Urine Alcohol Test is able to detect alcohol use even 3-4 days after it was used. Urine is collected and sent to SOS approved lab, results available in 2-3 business days. This option is ideal for clients who are to abstain from alcohol.

IMMEDIATE RESULT TESTING KITS (RESULTS IN 5 MINUTES)

These kits are specifically designed to help concerned individuals, parents, caregivers, family members and/or sober living houses screen for substance use with their children, teenagers, partners and house members. Each kit can test: • Available tests include: *K2, AMP, M-AMP, BAR, BZO, COC, MDMA, MTD, OPI, OXY, PCP, TCA and THC. • Results in 5 minutes, FDA 510(k) cleared to market and NIDA 5 cut-off levels meet SAMHSA guidelines The hair follicle testing consists of a 5 or 10 panel option, which tests for (alcohol, amphetamines, cocaine, opiates, (PCP), and marijuana). Additional drugs can be tested, if requested, can detect regular drug use for up 90-days. ETG- HAIR FOLLICLE TESTING (RESULTS IN 2-3 DAYS) Ethyl glucuronide (EtG) is a direct metabolite of ethanol alcohol. The presence of EtG in the hair, urine or blood can be used to detect recent alcohol consumption, even after the ethanol alcohol is no longer measurable. The presence of EtG in the urine, on the other hand, demonstrates that ethanol alcohol was ingested within roughly 80 hours after the ethanol alcohol has been metabolized by the body. In a hair sample detection is up to 3 months. All testing procedures are observed by SOS staff (except under special circumstances) HAIR FOLLICLE TESTING (RESULTS IN 2-3 DAYS)

Substance ABUSE SOS offers adult and adolescent programs on a sliding scale payment plan; which is meant to ease the financial burdens of each client.

AMETHYST HOUSE (sober living environment) is for any woman who you believe can benefit from a sober living environment. The house has 6 openings and is open to any women who are looking for a safe, sober, structured environment. The house is fully furnished and allows women who are new in recovery to have a safe structured environment to live and participate in services (listed below). Woman can stay from 4 months up to 1 year while they are establishing a foundation and getting on their feet in their first year of recovery.

SOS Sober House for Women

As a bonus to the sober living environment each woman has the option to participate in Outpatient or Intensive Outpatient Treatment weekly, which is a special and unique addition to traditional sober living houses. • Weekly Outpatient Substance Abuse and/or Co-Occurring Counseling • Weekly Women’s Therapeutic Recovery Group • Weekly Recovery Support and Aftercare/Relapse Prevention Services • Case Management Services (includes help finding job, food stamps, etc.. as needed) • Weekly Self-Help Meetings (AA, NA, CA, Alanon and/or CR) • Participate in Random Drug Testing • $100.00 weekly (sober house only) • $125.00 (sober living plus Outpatient) • $160.00 (sober house plus Intensive Outpatient) • Rent pays for (rent, utilities and all of the services listed above). • Additional expenses (food/drinks, laundry supplies and toiletries). Early Intervention Program ASAM Level 0.5: (Intended for clients identified in the misuse and early stages of abuse) The early intervention program consists of group sessions designed for those people in the misuse and/or early stages of misuse or substance abuse, or those early in the stages of readiness for treatment. Families are included in the process when possible. 1-2 hours per week (6-12 weeks in length) SOS offers adult and adolescent programs on a

Outpatient Program ASAM Level I Outpatient: (Intended for clients identified in early to middle stages of addiction) The basic elements of the Outpatient approach are group, individual and family sessions (early recovery skills, relapse prevention, family education and social support) and 2-3 individual sessions delivered over 12-16 weeks of treatment. The approach is comprised of a combination of the following components. • 2-5 hours of treatment per week (adolescents) and 2-8 hours of treatment per week (adults) • Co-occurring Recovery Groups • Anger Management/Coping Skills Group • Gender specific Men or Women’s Recovery Group • Matrix Model Early Recovery Group, Relapse Prevention, and Family Support Group • Peer Support (aftercare) Groups/Recovery Principles- Recovery Support Group • Substance Abuse Education • Weekly Self-Help Meetings (AA, NA, or CR) • Trauma Recovery Group • Case Management Services

Intensive Outpatient Options ASAM Level II.1 Intensive Outpatient Treatment (Intended for clients identified in middle to late stages of addiction) This Matrix Model program is an intensive treatment episode lasting up to 12-18 weeks that offers additional individual, group, family and conjoint sessions. This more intensive program is designed for those who have been diagnosed with co-occurring disorders, severe dependence, concurrent medical condition, or otherwise need more intensive treatment. • 6-9 hours of treatment per week (adolescents) and 9-10 hours of treatment per week (adults) • Co-occurring Recovery Groups • Anger Management/Coping Skills Group • Gender specific Men or Women’s Recovery Group • Matrix Model Early Recovery Group, Relapse Prevention, and Family Support Group • Peer Support (aftercare) Groups/Recovery Principles- Recovery Support Group • Substance Abuse Education • Weekly Self-Help Meetings (AA, NA, or CR) • Trauma Recovery Group • Case Management Services

What Is SOS’s Opiate Addiction Program? SOS’s opiate addiction program is an intensive (medically supervised) outpatient treatment program. SOS’s program has been serving individuals for approximately 5 years on a limited basis and is now expanding its capacity to be able to serve the needs of our community. The program allows individuals to move from their current level of drug usage to being completely free of any addictive opiate on an outpatient basis. This highly effective and intensive treatment regimen includes three components.

About Suboxone and Its Use Suboxone™ is the first opioid medication approved under the Drug Addiction Treatment Act 2000 (DATA 2000) for the treatment of opiate addiction and dependence, in an office- based setting. Suboxone™ (and its sister drug Subutex™) can also be dispensed for take- home use, just as any other medicine for the treatment of other medical conditions.

Medically supervised stabilization at the right level of treatment medications along with structured use of the medication. Intensive individual, group and family therapies to carry out an 16-20 week individualized treatment program, in order to get back your health, your job and your family. Continuing outpatient aftercare focusing on being completely drug-free by employing comprehensive counseling, therapy and state-of-the-art medications. individuals who have become dependent on or are addicted to opiates and whose goal is to abstain completely from all drug usage. SOS’s program integrates highly effective counseling with state-of-the- art medication treatments Buprenorphine (Suboxone). This treatment program is designed primarily for

The primary active ingredient in Suboxone™ is buprenorphine. Buprenorphine is a medication that can both activate and block opioid receptors. Depending on the conditions, partial agonists can produce effects similar to those of either agonists or antagonists. In combination with naloxone, it creates the opiate addiction treatment drug, Suboxone® and combined with naltrexone, it creates the opiate treatment medicaton Subutex®. Because buprenorphine is a partial opioid agonist, its opioid effects are limited compared with those produced by full opioid agonists, such as oxycodone or heroin. Suboxone™ also contains naloxone, an opioid antagonist.

The naloxone in Suboxone™ is there to discourage people from misusing the drug (i.e., dissolving the tablet and injecting it.) When Suboxone™ is used as directed (by placing the tablet under the tongue and allowing it to dissolve) very little naloxone reaches the bloodstream, so the patient primarily feels are the effects of the buprenorphine. However, if Suboxone™ is injected, the naloxone can cause that person to quickly go into withdrawal.

SOS has assembled a highly experienced team of professionals to provide intervention, consultation, case management and recovery mentoring services. We provide expert direction and planning for every aspect of the intervention, treatment and long term recovery process. There are many common misconceptions about intervention. Some think it is an emotional ambush or attack. But Love First demonstrates that intervention is a carefully planned process founded on love and honesty. Many people have heard about tough love where tough comes first, but the staff at SOS put love first.

Our clinical intervention services are comprehensive. Here are some of the elements of our structured intervention service, based on the best-selling book Love First, by Jeff Jay and Debra Jay: • Training, including how to answer objections, how to use the power of the group to break through denial, how to structure bottom lines and more. • Rehearsal, including various scenarios that might take place in the intervention, contingency planning and direction. • Intervention, a specific and organized conversation with the addicted person to break denial and move them into treatment. • Escort, there is no additional charge to escort your loved one to treatment, beyond the necessary travel expenses. • Follow-up, including liaison between the treatment center and the family and general questions. Case management services also available.
