a. A portfolio of educational accomplishments. This portfolio shall provide documentation of excellence and effectiveness in teaching and/or mentoring, including evaluations from students, residents, and peers. The evaluation methods should be clearly defined and described by the individual department in a formalized manner. In addition, the portfolio shall, when relevant, provide documentation of excellence in educational scholarship. Weight will be given to the development and organization of new and innovative teaching or evaluation methods, the restructuring of clinical services to include a greater educational component, course or resident rotation organization and administration, as well as participation on departmental, college, and national education committees. For mentoring, weight will be given to funding obtained for mentorship; to service as mentor on others’ mentored research grants; to local or national mentorship awards received; to workshops on mentorship conducted; and to letters of support from current or past trainees whom the candidate has mentored. b. Materials allowing evaluation of the candidate’s contributions to service, administration, and other university, community and national efforts. C. Track Switch Guidelines and documentation for a track switch are the same as that required for appointment/promotion to the track and rank for which the individual is being reviewed. For example, if the request is to switch from Associate Professor of Clinical (department Name; non-tenure track) to Associate Professor on the tenure track, then the same guidelines and documentation as described for promotion to Associate Professor must be submitted.
