5.0413.03 POST EXPOSURE EVALUATION Issue Date: 5/8/97 First Aid Incident Reporting:

All first aid incidents involving the presence of blood or OPIM will be reported to the school principal or his/her designee before the end of each school day. The report will include the following information: 1. The names of all first aid providers who render assistance, regardless of whether personal protective equipment was used, 2. The time and date of the incident, 3. The description of the incident, 4. The presence of blood or OPIM materials, and 5. A determination of whether an exposure incident occurred. The report will be recorded on a log that is available to all employees and is provided to the school nurse upon request. Training programs for school designated first aid providers will include specifics of this reporting procedure. Hepatitis B Vaccination series will be offered and made available within 24 hours to all unvaccinated first aid providers who have rendered assistance in any situation, regardless of whether or not a specific “exposure incident” as defined by the OSHA Standard occurred.
