Wellstar CHNA Report: 3 Years Later

Live to the Beat. Powered by Wellstar. In 2022, the Wellstar CHNA highlighted that the top causes of mortality and hospital use in hospital service areas are largely related to chronic disease and lifestyle factors (i.e., heart disease, stroke, diabetes, COPD, cancer, and substance use). In 2024, Wellstar launched a new program called “Live to the Beat. Powered by Wellstar.” Wellstar serves as a partner of the Live to the Beat Campaign, a nationally recognized program led by the CDC Foundation and the Million Hearts Initiative, aimed to help adults take steps to prevent heart disease and stroke.

The Live to the Beat pilot utilizes a United Way of Greater Atlanta community health worker to serve as a health coach. The coach empowers 50 individual members of faith communities, identified through Wellstar’s Congregational Health Network, to be advocates in their own health journey. Over a 12-month period, the program supports the members through 8 essential “small steps” to sustainable heart health. Live to the Beat Partners:

100% rated the program 5 stars during a recent midpoint review

100% reported improvements in their awareness of heart health resources

100% agreed that the program strengthened their perception of Wellstar as a true partner in their health and well-being

59% reported improvements in their average blood pressure readings
