Wellstar CHNA Report: 3 Years Later

Patients identified as having depression are referred to a behavioral health specialist. Behavioral Health Specialists are being embedded in the Wellstar pediatric practices over time to increase access. In fiscal year 2024, Wellstar embedded a behavioral health specialist at Wellstar Towne Lake Pediatrics. Medical Home Access In the 2022 CHNA Implementation Plan, Wellstar committed to guaranteeing that every newborn born at a Wellstar facility has a Pediatric Medical Home, is seen within 24-48 hours, and receives timely completion of 0-30 months well visits. All newborns born at a Wellstar Hospital can be seen at any Wellstar Pediatric Practice if the parent chooses. All Wellstar Pediatric Practices monitor WellChecks to ensure that patients are seen timely and receive appropriate vaccines. If a patient misses an appointment, the staff will contact the parent to attempt to get the child in for the missed appointment.
