Primary Care is busy, identify pinch points and help address these and enable collaborative working.
NNPC is acting as a facilitator in this project between North Norfolk GP practices (represented initially by Practice representatives) and Real World Health (who are ‘ building ’ the dashboard).
The aim is that when logged into the dashboard, Practices will be able to see certain metrics of their own Practice data, data across their PCN, and an overall North Norfolk picture of the ‘system’ . 3.2.2 Monday.com NNPC plan to implement the Monday.com platform over April / May 2023 to manage all aspects of customer relations, potential projects, as well as the project management processes. This will replace the former project management process of using Excel spreadsheets to track project progress. It will provide a standardised area for all of the project team and SMT to utilise. The platform is able to produce a significant amount of reports which will allow for easy updates and analytics to SMT to improve visibility of project workload and progress. It will ensure that quality and safety aspects are considered to give assurance that clinical and information governance frameworks are adhered to. 3.2.3 Staff Experience Survey – Core staff and clinicians Bespoke staff experience surveys will continue into 2023/24. The aim is that they will further aid responsiveness to staff wellbeing and will demonstrate organisational culture. They will provide the opportunity to celebrate the positive aspects, and to put measures in place to improve, where required. 3.2.4 Agilio TeamNet The aim is to introduce Team Net to NNPC over the course of April / May 2023, which will support and further enhance the monitoring of regulatory compliance with HR processes - for example monitoring mandatory training, policy & guideline updates. 3.2.5 Audit Schedule – Clinical Audits Clinical audit can drive improvement through a process of service review against expected standards, with the implementation of change as required. We aim to support and encourage our clinicians in clinical audits even further in 2023/24. We will plan teaching sessions and training in clinical auditing, and actively encourage clinicians to carry out audits by way of implementing improvements (where required), monitoring performance, quality of care, and aspects of patient safety.
3.3 Patient Safety & Quality Incident Reporting
NNPC is committed to continuous improvement, which includes learning from incidents & accidents, significant events, and serious incidents. We have robust reporting process which allows us to investigate, respond to, and learn from events in a way that improves clinical service delivery.
