3. For the Change Reason, select Birth / Adoption / Legal Guardianship of Child from the drop-down menu. Then enter the official date of birth as the Date of Birth, Adoption or Legal Guardianship .
4. Next, attach supporting documentation such as the official birth certificate/verification of birth or application for a birth certificate, then click Submit .
5. On the following page, click Open to start the benefit event.
