Example 2 Surveillance Officer McGruder witnessed the incident from the surveillance room. In his interview, McGruder stated that he immediately dispatched security to the scene and continued to monitor the incident. Security Officer Smith informed me that when he arrived, the customer was agitated and refused to leave voluntarily as he had requested. Thereupon, according to Smith, he placed his hand on Customer A arms in an effort to remove her from the Table. As he did, Customer A fell onto the floor. McGruder stated that while helping her to her feet, he noted a strong odor of alcohol and observed that her face was flushed and that her facial expression was consistent with intoxication. Smith and security officer Johnston escorted Customer A to the main entrance where she entered a cab and departed.

At approximately 7:00 p.m. on the night of April 17, 2016, the pit boss for Pit 2, Joe Doe, requested Customer A’s identification to verify her age. During my interview with Mr. Doe, he stated that Customer A refused to provide the information and became upset. He further stated that Customer A in a loud voice accused him of “picking on her because she was on a winning streak.” At that point, according to Doe, Customer A threw her cards up into the air and knocked over a cup of coffee, spilling it on the left hand of the customer to her right at position 4 of Table 5. (See diagram 1 attached) A review of the surveillance recording verifies Mr. Doe’s account of the incident.

Investigative Reports: Relevance

u Trickier than you may think… u All relevant information should be included in an investigative report u Irrelevant information should be excluded u The trick is in determining what IS and what is NOT relevant u As a general principle, relevant information is information reasonably related to the matter under investigation. u It is information (evidence) that is “probative” of the matter at issue. u A fact that is of consequence to the determination of the action
