3.5 Payroll Deductions Various payroll deductions are made each payday to comply with federal and state laws on taxes and insurance. Deductions will be made for the following: Federal and State Income Tax Withholding, Social Security, Medicare, State Disability Insurance & Family Temporary Disability Insurance, and other items designated by the employee or required by law (including a valid court order). Employees can adjust their federal and state income tax withholding by completing the proper federal or state form in UKG or submitting it to Accounting or Human Resources. At the beginning of each calendar year, employees will be supplied with their Wage and Tax Statement (W-2) form for the prior year. This statement summarizes the employees’ income and deductions for the year. 3.6 Wage Garnishment A garnishment is a court order requiring an employer to remit part of an employee's wages to a third party to satisfy a just debt. Once the Company receives the legal papers ordering a garnishment, we are required by law to continue making deductions from the employee’s check until we have withheld the full amount or until we receive paperwork from the court to stop the garnishment. Even if the employee has already paid the debt, we still need the legal papers to stop the garnishment.
3.7 Direct Deposit Midwest Fastener Corp. offers two options for its employees to receive their wages. Employees may enroll in direct deposit using their financial institution or elect to have wages electronically deposited onto a pay card. 3.8 Rehire, Layoffs, & Recalls Rehire Midwest Fastener Corp. employees who have terminated their employment may be eligible to return based on their overall performance. Layoffs & Recalls Midwest Fastener Corp. employees with ninety (90) days of service or more retain their seniority date, vacation allotment, and original hire date, provided the layoff is six months or less. Employees whose layoffs exceeded six months or more will be treated as a new hire for benefit purposes. Midwest Fastener Corp. employees with less than ninety (90) days of service are considered permanently terminated when the time of service equals the time of layoff (in days).
