4.2 Attendance Punctuality and regular attendance are essential to the successful operation of the Company's business. If an employee is unable to report to work (or to report to work on time) for any reason, the employee must notify their supervisor before their starting time. If an employee desires to leave work for any reason during the workday, the employee must obtain the approval of their supervisor prior to leaving. Excessive absenteeism or tardiness may subject the employee to disciplinary action, up to and including termination. Employees are required to follow established guidelines for recording their actual hours worked.

Deficiency Point System for Nonexempt Hourly Employees Point System Each employee will be allowed a total of nine (9) attendance deficiency points within a floating six (6) month period. Absence
• ABS – 1 Point Employee notifies the Company of the employee’s inability to report for work prior to the start of the scheduled shift, but does not receive approved personal time (PTO/UTO). • AAS – 2 Points Employee notifies the Company of the employee’s inability to report for work within the first four (4) hours of the start of the scheduled shift. • AWN – 3 Points Employee notifies the Company of the employee’s inability to report for work after the first four (4) hours of the start of the scheduled shift, but prior to the end of their scheduled shift. • NCS – 4 Points Employee fails to report to work or notify the Company during the scheduled work day. • LOA – 0 Points Employee receives pre-approval from management for a leave of absence of consecutive days off which are un- paid and are not applied as credited hours towards full time employment. • FML – 0 Points Employee may take up to 12 weeks of unpaid medical leave under the
A missed clock in/out is a violation of this policy and includes
• Failure to clock in/out at the designated time clock at the beginning and/or end of the assigned shift • Failure to clock in/out on designated time clock for the meal break (when clocked in- all employees must remain on premises, when clocked out for meal breaks employees are permitted to leave the premises) • Failure to accurately and timely report time worked • Clocking in/out early (or late) for assigned shift without prior approval • Clocking in/out for another employee If an employee works anything other than their scheduled hours they need to have the starting and stopping time pre-approved by their supervisor. Midwest Fastener Corp. does allow employees to clock in/out 5 minutes before/after a scheduled shift without earning attendance deficiency points. Exempt (salaried) employees are expected to work the scheduled shift assigned to the employee with an average of 40 hours per week. To fulfil the work obligation, exempt (salaried) employees may need to be willing to work beyond Midwest Fastener Corp.’s regular business hours. Since exceptions to the general
