4.2 Attendance and Punctuality General Policy Punctuality and regular attendance are essential to the successful operation of Midwest Fastener Corp.’s business. Employees are expected to report to work on time and be prepared to perform their duties. • If unable to report to work or arrive on time, employees must notify their supervisor before the scheduled start time. • Call-offs need to be done two hours before the start of your shift to avoid getting attendance points. • Employees seeking to leave work during the workday must obtain prior approval from their supervisor. • Excessive absenteeism or tardiness may result in disciplinary action, up to and including termination. Timekeeping Employees must accurately record actual hours worked and follow established guidelines: • Clocking In/Out Employees must clock in/out at the designated time clock at the beginning and end of their shift. • Meal Breaks Employees must clock in/out for meal breaks. When clocked in, employees must remain on Company premises; when clocked out for meal breaks, employees may leave the premises. • Accuracy All time worked must be accurately and timely reported. • Prohibited Practices Clocking in/out early or late without prior approval, or clocking in/out for another employee, is not allowed. • Overtime Approval Any work outside scheduled hours must be pre-approved by the supervisor. • Grace Period Employees may clock in/out within 5 minutes before or after their scheduled shift without incurring attendance deficiency points.
Exempt (Salaried) Employees • Exempt employees are expected to work their assigned schedule, averaging 40 hours per week. • Work may occasionally extend beyond regular business hours to meet job requirements. • While exempt employees have flexibility regarding schedule exceptions, patterns of attendance abuse will be addressed and may result in disciplinary action. Deficiency Point System for Nonexempt Hourly Employees Each employee will be allowed a total of nine (9) attendance deficiency points within a floating six (6) month period. Points are assessed as follows:
Absence
• ABS – 1 Point Employee notifies the Company of the employee’s inability to report for work prior to the start of the scheduled shift, but does not receive approved personal time (PTO/UTO). • AAS – 2 Points Employee notifies the Company of the employee’s inability to report for work within the first four (4) hours of the start of the scheduled shift. • AWN – 3 Points Employee notifies the Company of the employee’s inability to report for work after the first four (4) hours of the start of the scheduled shift, but prior to the end of their scheduled shift. • NCS – 4 Points Employee fails to report to work or notify the Company during the scheduled work day. • LOA – 0 Points Employee receives pre-approval from management for a leave of absence of consecutive days off which are un-paid and are not applied as credited hours towards full time employment. • FML – 0 Points Employee may take up to 12 weeks of unpaid medical leave under the
Family Medical Leave Act. Time off is applied as credited hours towards Full-Time employment.
