Anyone who violates this policy will receive disciplinary action (per the discipline and conduct policy) and sent home with attendance occurrence, without pay, to change into acceptable attire and return. Any further violations will lead to disciplinary action up to and including termination. 4.6 Employee Badges Facilities with a Badge System All employees, staff members, and office holders are issued a photo Identification Badge and are required to always wear it. The badge must always be visible while in the building. If an employee loses their ID Badge, it must be reported immediately to their Supervisor/ Manager, and it will be disabled in the system. Any damaged Identification Badges that will no longer open the door at the employee entrance must be replaced. There is a $10.00 fee for any replacement badges or any unreturned badges. All employees, staff members, and office holders will be given the Identification Badge policy and have on file an acknowledgement of their understanding of the full policy. 4.7 Visitors in the Workplace To provide for the safety and security of employees, visitors, and the facilities at Midwest Fastener Corp., only authorized visitors are allowed in the workplace. The family and friends of employees are discouraged from visiting unless prior permission is granted from the Operations Manager. All visitors must enter through the main reception area. Authorized visitors will be escorted to their destination and must be accompanied by an employee at all times. If an unauthorized individual is observed on Midwest Fastener Corp.’s premises, it should be reported immediately to a manager or supervisor, or the individual should be directed to the main entrance for assistance. All visitors in the building are required to sign in with the receptionist and wear a “visitors” badge at all times while in the building.

Unacceptable Business Casual attire is that which could appear to be unprofessional and would include

• Tank tops, warm-ups, scrubs • See-through or low-cut tops • Clothing that leaves the shoulders and arms bare, worn without a top that covers the shoulders • Athletic wear, wind suits • Shorts • Skirts above mid-thigh • Tight fitting skirts or slacks and skirts made of Lycra/spandex • Pants that stop at or above knee • Leggings can be worn with a top or skirt that covers down to mid-thigh • Flip flops

Casual Fridays While Fridays will be considered Casual Fridays, the above rules still apply with one exception: you may wear casual clothing. If you decide to wear casual clothing, make sure they are in good shape and appropriate for the office.

Any other clothing that could appear to be unprofessional should be avoided.

Warehouse/Factory Personnel Warehouse/Factory personnel are required to wear the following attire

• Bottoms worn at the waistline at all times while on Company property • Long or short-sleeved tops, tank tops that cover the torso, shoulders, chest, and waistline • Shoes that cover the entire foot • Safety glasses and other PPE/ safety equipment for specified work area recommended

Each department supervisor/team leader is responsible for the enforcement of the dress code. If there is a question about what is or is not acceptable, check with the department supervisor, team leader, or management.
