Emergency Medical Treatment In the case of any debilitating illness or injury to an employee, even within the normal range of pre-diagnosed conditions or illness, it is our policy to immediately call 911 Emergency Services on the employee’s behalf. Safety is our main concern and immediately following the call for emergency services, the emergency contact that was provided by the employee will be notified of the situation. It is the employee’s right to refuse any medical services offered by the attending medical staff on the scene when they arrive. However, a document from an attending physician must be provided to the Company upon the employee’s return to work. This document must show proof that it is safe for the employee to return to work and must contain in writing any detailed restrictions the employee has and for how long. If restrictions are necessary and it is a non-work-related accident or incident, reasonable consideration will be given to the employee’s request to return to work with limited capacity. The employee will be notified in person, by phone, or in writing about the Company’s decision. Hazard Communications Program The Company’s objective is to comply fully with the standards known as “Hazard
Miscellaneous Tools Tools required by factory personnel while performing their job will be supplied by the Company. Any broken tools will be replaced, however, if the damage is caused by carelessness or negligence, the Company may require the employee to pay the cost of replacing the tool. No tools are to be taken from the premises at any time. Personal Protective Equipment Midwest Fastener Corp. will provide personal protective equipment (PPE) as called for by the hazard assessment for each operation. PPE is available on request and will be provided by the Company as needed. However, it is the employee’s responsibility to provide prescription safety glasses. Instructions on proper use of the PPE will be given as the job requires. Please see your supervisor if you have further questions regarding the use, adjustment and wear of personal protective equipment. Proper care during its use should be monitored, and new PPE should be issued as needed. Dispose of all PPE in the nearest receptacle after its useful life. No special disposal is required. Fire Procedures The goal of the Company during a fire emergency is to keep all employees safe and maintain an orderly exit from the building. The Company has an Emergency Action Plan (EAP), available upon request to employees and any OSHA officials when requested. Floor diagrams of the entire plant are posted on company bulletin boards. In the case of a fire alarm, all employees are to immediately proceed to the nearest exit. All employees will then report to the designated facility evacuation area where supervisors/managers will take attendance to ensure all employees are accounted for. At no time should anyone re-enter the building for any reason until it is deemed safe and cleared by management or safety personnel – this includes entering the building for the retrieval of any personal property.

Midwest Fastener Corp. has assigned direct responsibility for compliance with its Hazard Communication Program to its supervisors and managers. You will be given formal safety training on your first day of employment. The New Hire Safety Training and Orientation packet will be reviewed with you by the Human Resources Department and/or your manager/supervisor. Safety is something every employee should take pride in, and each employee is encouraged to take an interest and actively participate in our training.
