Miscellaneous Tools Tools required by factory personnel while performing their job will be supplied by the Company. Any broken tools will be replaced, however, if the damage is caused by carelessness or negligence, the Company may require the employee to pay the cost of replacing the tool. No tools are to be taken from the premises at any time. Personal Protective Equipment Midwest Fastener Corp. will provide personal protective equipment (PPE) as called for by the hazard assessment for each operation. PPE is available on request and will be provided by the Company as needed. However, it is the employee’s responsibility to provide prescription safety glasses. Instructions on proper use of the PPE will be given as the job requires. Please see your supervisor if you have further questions regarding the use, adjustment and wear of personal protective equipment. Proper care during its use should be monitored, and new PPE should be issued as needed. Dispose of all PPE in the nearest receptacle after its useful life. No special disposal is required. Fire Procedures The goal of the Company during a fire emergency is to keep all employees safe and maintain an orderly exit from the building. The Company has an Emergency Action Plan (EAP), available upon request to employees and any OSHA officials when requested. Floor diagrams of the entire plant are posted on company bulletin boards. In the case of a fire alarm, all employees are to immediately proceed to the nearest exit. All employees will then report to the designated facility evacuation area where supervisors/managers will take attendance to ensure all employees are accounted for. At no time should anyone re-enter the building for any reason until it is deemed safe and cleared by management or safety personnel
Off-Duty Social & Recreational Activities During the year, employees may participate in social or recreational activities sponsored by the Company. An employee’s attendance at such social activities, however, is completely voluntary and is not considered work-related. Midwest Fastener Corp., nor our insurers, will be liable for the payment of workers’ compensation benefits for any injury that arises out of an employee’s voluntary participation in any off-duty and/or Company-sponsored recreational, social or athletic activity that is not part of the employee’s work-related duties. Recreational walking in the factory or warehouse is prohibited. 4.13 Injuries All injuries must be reported immediately to a Midwest Fastener supervisor, regardless of how insignificant the injury may appear. If an employee goes to the emergency room or to any doctor without authorization, the employee will be liable for any medical bills associated with unauthorized medical treatment. Drug screening is mandatory for all work-related injuries and accidents. Midwest Fastener Corp.’s will attempt to have available, when approved by medical practitioners, a light duty return to work program. Although the employee on limited or light work duty may not be in the department, they were working previous to the injury, a suitable task may be provided within the Company. Health-Related Issues Employees who become aware of any health-related issue should notify their supervisor of any health status changes. This policy has been instituted strictly to protect employees. A written “permission to work” from the employee’s physician is required at the time or shortly after notice has been given. The physician’s note should specify whether the employee is able to perform regular duties as outlined in their job description. A leave of absence may be granted on a case-by-case basis. If the need arises for a leave of absence or Family Medical Leave Act (FMLA), employees should notify human resources.
