WALKER BOOTS Which Boot Is Best?

Walker Boots are one of the most commonly used orthopaedic products in the hospital, with a wide range of indications. There are multiple types of boot, which are better suited to specific injuries. Ultimately, the primary aim of a ‘walking boot’ is to immobilise the ankle, reducing plantar and dorsiflexion whilst also limiting inversion and eversion. This protects the joint and reduces risk of further injury, allowing a safe environment for the bone or soft tissues to heal. Secondly, walker boots allow optimal loading to take place, where the patient can start putting weight through their ankle which can encourage earlier strengthening of the injury site and faster recovery ₁. There are other important considerations too; for example, the height of the boot needs to be as low as possible to minimise disruption to the patient’s walking pattern, and it needs to be durable enough to withstand day-to-day use. Equally, the ease of use for you as clinicians is key, and the simple fitting and adjustment of walker boots ensures you can allow efficiency in your service. Aircast is one of our oldest brands, and the first company to introduce a ‘pneumatic’ walker boot, introducing air cells to provide comfort, superior immobilisation, and crucially, swelling reduction. Together with our ProCare brand, we have a complete range of walker boots, to provide your patients with the optimal boot for their injury.

PNEUMATIC WALKERS Pneumatic Walkers include multiple air cells, and are best for injuries where the patient will be in the boot for a longer duration; for example, achilles ruptures, severe sprains and fractures. The Aircast AirSelect Walkers are our most popular. They include key technology to provide optimal comfort and stability, and use our duplex aircells to reduce swelling.

BASIC WALKERS Basic, or ‘Stick and Foam’ Walkers do not include aircells, but are economical choices when the patient will be in the boot for a shorter period of time. WALKER BOOTS Using Walkers Effectively?

INDICATIONS Due to the difference in technology and indication between pneumatic and basic walker boots, there is naturally a cost variance.

We understand managing your budget is important, therefore ensuring the correct boot is used for the appropriate indication is a key consideration. For example, using pneumatic walkers for a low level ankle sprain is likely not necessary. This is not a cost effective solution, and a basic walker is better suited. In comparison, more serious indications such as a grade 3 ankle sprain or fracture where the patient needs longer term immobilisation may be more effectively managed in a pneumatic walker₃.

Based on the usage of multiple large hospitals in the UK, we have provided the following data to help you determine how your service compares.

AVERAGE WALKER BOOT SPLIT: NHS The 40/60 split is a common trend across hospital sites and is a good indicator of the volume split expected in the NHS. The below chart includes an outlier (Hospital B) compared to the general average. This suggests patients may be being inappropriately fitted in a basic walker. This may lead to poorer outcomes, multiple return visits to clinic, and if using for a prolonged period – multiple boots per patient. Ultimately, a less cost-effective option, even if the initial cost of the basic walker is less.

FITTING ADVICE FOR CLINICIANS

Fitting Walker Boots effectively and efficiently is an important process. This ensures the injury site is appropriately protected and allows for improved patient comfort.

FITTING TIPS:

• Choose the correct style of walker, based on the injury • Fit the walker boot in a seated position • Ensure correct sizing as this prevents slipping of the foot inside the walker • Aim to fit the heel as far back in the walker as possible • With pneumatic walkers, ensure the aircells are inflated appropriately • Ensure the patient understands how to inflate, and deflate the aircells on their pneumatic walker

WHEN TO WEDGE

For Achilles Tendon Ruptures, pneumatic walker boots are used alongside Heel Wedges to increase Plantarflexion at the ankle and reduce the load on the repairing tendon. Hospitals will have their own protocol with regards to Achilles Ruptures, but our advice on wedges (our older style) is below: • Ensure the correct size wedges are used (Regular Aircast wedges fit XS, S, M, L but XL Wedges need to be ordered separately) • Wedges are Left OR Right, ensure the correct side is used (Note - our new wedges are universal)

• We advise that 3 Wedges are used, based on the size of boot: XS/S = Smallest 3 wedges | Medium = Middle 3 | L = Largest 3 • Angle of Wedges: 3 = 22 Degrees | 2 = 16 Degrees | 1 = 10 Degrees

ANKLE BRACES Support at Every Step

Ankle Injuries are one of the most common. With this brings many patients presenting with ankle sprains that vary in severity. Whilst the higher grade of sprains and syndesmosis injuries are more commonly managed using a pneumatic walker boot, lower grade acute ankle sprains can also benefit well from ankle braces ₃.

Historically, these have been rather bulky and whilst they provided stability, often didn’t fit into a ‘regular’ shoe or trainer. These have developed over the years to provide a better balance of stability and sleekness.

Whilst walker boots certainly provide better protection, ankle braces are a better functional option as they allow the patient to load the ankle joint, encouraging strengthening of the surrounding structures and earlier development of proprioception. Their lightweight and low profile design makes them far more suitable for day-to-day use; certainly for conditions such as Chronic Ankle Instability.

Our Aircast A60 is arguably the most well known ankle brace amongst clinicians. Gaining its reputation through sports people, it has been used throughout the NHS for over 10 years. LIGAMENT BRACES Where Stability Matters

Ligament or ‘Functional’ braces have been utilised by clinicians for decades. In 1984, the AAOS (American Academy of Orthopaedic Surgeons) sponsored a seminar on knee braces and identified 3 key areas of braces: Prophylactic, Functional, and Rehabilitative (ROM style). Ligament braces fall into the ‘Prophylactic’ and ‘Functional’ categories. Their function is to provide stability to the ligaments that have been injured, allowing patients to return to their chosen activity with confidence and help prevent further injury.

These braces vary in their levels of stability. An ‘SK’ style knee brace can be used for low level injuries where patients need very mild support and compression. These require robust materials and ease of fitting to allow for simple everyday use.

FOUR POINTS OF LEVERAGE The Four Points of Leverage System™ is a patented, dynamic bracing system. Simply, it puts a posterior pre-load on the tibia. But what exactly are the four points and how do they work?

DonJoy 4-Points-of-Leverage knee braces are worn either prophylactically or post-surgically to protect the ACL. 4-Points-of-Leverage refers to 4 specific points on the brace; the rigid anterior thigh cuff, rigid posterior calf cuff, posterior thigh strap, and anterior calf strap. Each of these points work together to apply a posterior force on the lower leg; reducing the amount of strain on the ACL. These types of braces are rigid and either off the shelf - made of aircraft grade aluminium and adjustable to fit each individual - or custom made of carbon fibre and specifically designed for the individual. Braces utilising 4-Points-of-Leverage technology significantly reduce ACL Strain₆ which is particularly important during the rehabilitation phase as the ACL graft is remodelling. Rigid braces not only provide ACL support but they also protect additional ligaments that support the knee; MCL, LCL and PCL. FOURCEPOINT HINGE Found only on our ACL specific braces, this reduces the amount of time the knee is in an “at risk” position, enhances the 4-Points-of-Leverage by increasing the posterior load applied by the brace on the tibia, and improves a patient’s natural gait by applying progressive resistance before reaching a firm end point during movement.

ROM CONTROL Our DonJoy rigid braces can be adjusted to provide specific ROM control, to allow for more precise rehabilitation protocols.

CUSTOM BRACING (DEFIANCE PRO ONLY) Our DonJoy Defiance® Pro is custom made to your patient’s exact requirements. These can incorporate strapping and hinges specific to a range of indications including OA/Offloading, ACL, PCL, and Quadriceps Deficiency.

OFFLOADER BRACES

Are you Providing ‘SAFE’ Offloading?

Offloading knee braces have grown significantly in their use over the last 5 years. The number of patients presenting with Knee Osteoarthritis is increasing, resulting in lengthy waiting times for surgery and an increased focus on non-surgical management ₄. Designed to reduce the pressure through the affected compartment of the knee, offloader braces are available in various designs and concepts. Considered a cost-effective option for managing the symptoms of Knee OA, offloaders play a key role in helping patients. Interestingly, the NICE guidelines suggest that patients shouldn’t actually be provided with a device such as a brace, unless they meet the following criteria: • There is joint instability or abnormal loading AND • Therapeutic exercise is unsuitable or ineffective without the aid of a device, AND • The addition of a device is likely to improve movement and function

ROM BRACES The Most Difficult Braces to Fit?

Knee ROM (Range of Movement) Braces are one of the most commonly used products within hospitals. Their primary function is to protect the knee, limiting range of movement and allowing the patient to rehabilitate their injury in a safe environment. This allows for effective pain management and strengthening of the joint. The injuries they are used for are wide ranging, and patients can be using them for anywhere between 2 to 12 weeks, depending on the severity of injury. Exact protocols are often specified by the prescribing clinician, so it is important the brace can accommodate. The most important factor is to ensure a great fit. A bad fitting ROM brace can be inconvenient for a patient and therefore ineffective. They need to be simple to fit (for patient and clinician), easy to live with, and of course provide the stability and ROM control to allow effective injury management. Historically, ROM Braces can be tricky to fit for clinicians, resulting in poor fitting, slipping, and patient inconvenience. Therefore it is important to be confident in the ROM Brace you use, and have all the information required to ensure the best possible fit for each patient. Our brand DonJoy is arguably our best known, mostly for knee braces. We started in 1978 in a Californian garage with two friends. They began by creating a basic neoprene sleeve, and named the company after their wives Donna and Joy. DonJoy was born, and with over 45 years of innovation we continue to provide clinicians with products they can trust.

ELBOW BRACES Whilst less frequently used than Knee ROM Braces, Elbow ROM braces remain a useful solution for specific indications. Providing effective ROM control for injuries such as biceps tendon repair or humeral fractures, the X-Act ROM Elbow is designed to provide comfort and stability to your patients. With a similar design to our Knee ROM solutions, the X-Act ROM Elbow uses telescoping contact points and malleable cuffs to provide a custom fit to your patient’s’ anatomy. ISOFORM® POSTURAL EXTENSION

SPINOSTRAP

WHAT’S THE DIFFERENCE? The Spinostrap is an entry level semi-rigid brace to provide support and kyphotic postural correction. It is slimline and designed to be a lighter weight alternative to the larger, more stable options of the TLSO or Postural Extension Brace. The IsoFORM TLSO provides trunk rotation and stability from T3 – S1 in 3 planes of motion. The anterior, posterior, and lateral panels, together with a telescopic sternal bar, provide optimal stabilisation and comfort. Indicated for patients with compression fractures of the thoracic spine, osteoporosis and kyphosis.

The Isoform Postural Extension provides thoracic stability to aid conditions such as kyphosis, osteoporosis and mild scoliosis.

Both the TLSO and Postural Extension braces are designed to be ‘semi-universal’, with only 2 sizes to help reduce your stock requirements and save space.

COLD THERAPY Tried and Tested Combination Therapy

One of the biggest challenges facing the orthopaedic wards within the NHS is the waiting lists that patients face to access surgery. Therefore, length of patient stay and time to discharge are extremely important. Following surgery, there are several limiting factors that can slow down a patient’s recovery. There are two we can assist with:

1 – Swelling – This limits range of movement and functional movements 2 – Pain – Results in reduced muscle activation and limits functional activity

Cold Therapy has been used in the management of injury for a number of years. The acronym ‘RICE’ (Rest/Ice/Compression/Elevation) was first introduced in 1978 and has evolved over the years, most recently to ‘PEACE & LOVE’ ₇.

CRYO/CUFF AND ICEMAN
