My experience with MaiLi and Ellansé

Dr Ahmed El Muntasar of The Aesthetics Doctor, award-winning GP

"With MaiLi and Ellansé, the future of facial rejuvenation is brighter than ever, offering patients the opportunity to age gracefully and beautifully, one syringe at a time."

WHY DO YOU OFFER MAILI IN YOUR CLINICS?

WHAT IS THE DIFFERENCE BETWEEN MAILI PRECISE, DEFINE, VOLUME AND EXTREME? Essentially, MaiLi has got four main products. Precise, Define, Volume, and Extreme. And the difference is the G prime, or essentially how thick the product is. For example, around the tear trough area, you want it to be nice and thin, this is why you use Precise. And for areas like the chin or the jawline, you want it to be quite thick to be able to reposition tissues, to be able to lift and define those areas, and this is why we use Extreme. For the areas around the mouth, we like it to be nice and soft, but still causes noticeable improvement, and this is why I use either Volume or Define.

Over the years I've used a variety of different dermal fillers. I am always out there on the hunt to find the best products to be able to achieve for my patients. To be honest with you, when I first came across MaiLi, I just wondered if it's just another dermal filler, it's another “best product” out there, but the reality is after I tested it over periods of months, I found the results are incredible compared to anything else and this is why I'm a complete convert. I only use MaiLi in my clinics now. For me, it represents the pinnacle of hyaluronic acid-based dermal fillers, offering unparalleled versatility and efficacy.

"Achieving natural-looking results is a top priority for my patients when they are seeking aesthetic enhancements."

There are so many different great products in the range that they offer, and I have had a lot of support from them. Over the years I have text, emailed, messaged all the team asking questions, requesting data and studies; everyone wants to support, everyone wants to be there for you, so I cannot recommend them enough.

TELL US ABOUT YOUR EXPERIENCE OF USING MAILI AND ELLANSÉ? Achieving natural-looking results is a top priority for my patients when they are seeking aesthetic enhancements. The combination of MaiLi and Ellansé allows me to achieve precisely that by restoring facial volume and improving skin quality in a gradual and subtle manner. Rather than creating an artificial or overfilled appearance, this synergistic approach enhances the patient's natural beauty, preserving their unique facial features while addressing specific concerns. The result is a refreshed and rejuvenated appearance that looks entirely authentic and age appropriate. With MaiLi and Ellansé, the future of facial rejuvenation is brighter than ever, offering patients the opportunity to age gracefully and beautifully, one syringe at a time. WHAT HAS YOUR EXPERIENCE BEEN LIKE WORKING WITH SINCLAIR? So, my experience with Sinclair goes back to about 2018, I have been working with the company for many years. I absolutely love working with them, it's like a big family!

ABOUT DR AHMED EL MUNTASAR

Dr Ahmed El Muntasar is an award- winning GP and aesthetics Doctor with clinics in London, Leeds, and Cheshire. WHAT DREW YOU TO USING ELLANSÉ?

It was actually the first Sinclair product I used from the Sinclair portfolio, after I saw the results of a fellow doctor and thought it looked beautiful; the volumising effect, the natural enhancements and improvements in the skin quality were second to none. I have found with Ellansé providing results lasting up to 24 months (with Ellansé M) it empowers my patients to choose the duration of their rejuvenation according to their preferences.

restricted and could perform treatments on tanned skin. When Boris announced a nationwide lockdown and businesses temporarily closed, I admit I must have cried for a good 12 hours, scared, confused and not knowing what was going to happen next. Then I had a lightbulb moment. This was going to be the time we needed to make the changes with our machines. The rest is history. As time has gone on... "...I can verify that the Elysion Pro laser hair removal machines - or as I call them ‘the workhorses of the business’ are hyper reliable." Our machines are switched on 24/7 with back-to-back appointments. The diode machines are a great all- rounder, ensuring optimal results and a virtually pain-free experience. There’s a reason they are rated the best in the industry for their power, precision and versatility. We also have great service agreements in place with Sinclair, where they will guarantee to fix or replace a machine should anything happen, which is great for our business and causes less business interruption. No more last-minute trips to London for repair! If I could give anyone the best advice when it comes to choosing machines I would say, always do your research into the finer details, such as what happens when a machine breaks and also what additional support you will receive such as training. And always check with your insurance what level of training they require you to have, as legislation changes all the time.

FAST FORWARD TO 2024 AND WHAT DOES LASERHQ OFFER? Fast forward to today, we've just celebrated our 7th birthday, and the business is busier than ever. We now have five clinics, a training academy and a supplies business! We have just opened our second location in Liverpool at Harvey Nichols. It’s amazing to see our brand sitting alongside such a well-known luxury brand. We’ve also introduced new service offerings at all our clinics including laser tattoo removal, blood work analysis, hormone testing services and collaborations with Hertility Health and the Endometriosis Foundation to name a few! We are definitely not your average laser brand and like to offer our clients so much more. We have helped so many people on their journey and changing lives for the better is what we do and love the most! We also raise lots of money for our charities such as the Endometriosis Foundation and drive awareness and help to break down the stigmas attached to Endometriosis, PCOS and hormone health with our annual #breakthetaboo campaign. “I am motivated and passionate about what I do, and I am hugely driven by the idea of making a difference in people’s lives.” My businesses always come first and I work on them every day. Taking the time to work on your business daily leads to greater success. I seek out any problems, make things better by improving on our processes and practises, and grow and build relationships with people.

Helen Quayle, Founder – LaserHQ

M ost people curse the business. But not me. This was the break that my business needed to take a breath, evaluate the business strategy and make changes for the better! Once our doors were back open after the lock down, we were flat out and due to the reduced treatment times from our new laser hair removal machines, the Elysion Pro, we were able to double our clients across all our clinics. As a result, my business grew 400% within a year. Those were the words my pandemic or recall it as one of the worst times in accountant called to tell me. I was completely shocked and couldn’t believe it. We had turned a difficult business period into a positive one. So how did it happen?

It's crazy to think we now have 20 members of staff across all our clinics, with plans for additional hires. They are all incredibly hard-working and I take their professional development seriously, with incentives, pay rises and promotions. Putting the right team in place has always been an essential element of LaserHQ’s growth. It has enabled me to have the right support in place to grow the business and drive forward the brand with the best team behind me. "I'm proud to be a business leader; and I am focused on creating a beautiful clinical environment for the clients and staff."

As my client list and reputation grew, I branched out to Liverpool. Working all hours around the clock I would lug my machine around in my car driving tirelessly up and down the motorway, to meet the demand! It took a good couple of months to build it up initially. Over time, and seeing the results that we were achieving with our clients with their ongoing treatment plans, I started realising how the service genuinely changed people’s lives. That’s when I really wanted to get the message out there. I used social media to really drum up awareness of my services and business offering. It was almost overnight that I noticed I was getting booked up. Before long I realised the workload was just too much for one person, and I employed my first staff member Vic, who helped me with bookings and admin. Vic is still with me today, supporting me and the business as always, and has progressed to my Finance and Operations Manager.

“I'm proud to be a business leader; and I am focused on creating a beautiful clinical environment for the clients and staff.” The business was in a really stable position and we had just had a full business restructure. The only weakness we had was our laser hair removal machines. Their reliability and repair terms meant that every time a head broke - which was a common occurrence in those times, I personally had to pack the machine into the boot of my car and travel to London for repair. This could sometimes take up to three days and caused maximum business interruption as we needed to then cancel and rearrange client’s appointments. With multiple machines across our clinics and wanting to expand and open further clinics, I knew that something had to give and we couldn’t carry on this way. But we didn’t have the time available to enable such a drastic operational change within the business! We came across the Elysion Pro by chance before the pandemic hit, and straight away we were impressed with not only the aesthetic look, but how easy they were to operate, the reduced treatment times, and of course their two functions of dynamic and static mode. It also meant we weren't as

BACKGROUND

Seven years ago, after being made redundant I decided to launch LaserHQ. I conceived the concept after I noticed a gap in the market for laser hair removal; I was interested in laser hair removal, looking for somewhere to have it done myself, but couldn't find any affordable options. The places that offered the service were either too pricey or didn't specialise in it. I realised that there wasn't a specific brand associated with laser hair removal. My vision from the get go was to be the best laser brand in the UK. I spent six months researching the best machinery and equipment and undertaking the specialist qualifications required to be a laser technician. It was quite a big risk to take and required substantial financial investment to first get the business off the ground. I built the company from scratch and started out in a tiny room in the back of a salon as a one man band, there were literally four walls, a bed and a machine.

WHAT’S NEXT FOR LASERHQ?

There’s so much on the cards. It's an exciting time for the business. We’ve got more growth plans including opening even more clinics, and we’re also looking to launch more services. The opportunities are just flowing in - we are really excited for 2024 and beyond!

A decade of artistry: the evolution of aesthetic practice with Silhouette Soft

particularly effective in treating fine lines and wrinkles, providing subtle yet noticeable improvements in facial contours. However, the lifting effects of PDO threads may not be as pronounced or long-lasting as those achieved with Silhouette Threads. In comparison, Silhouette Threads combine the benefits of both dermal fillers and PDO threads, offering immediate lifting effects along with long-term collagen stimulation. Their bidirectional design and dissolvable sutures ensure precise tissue repositioning and sustained improvements in skin laxity, making them a comprehensive solution for facial rejuvenation.

NO ONE TREATMENT FIT ALL CONCERNS:

CONCLUSION:

As I reflect on my journey with Silhouette Soft over the past decade, I am reminded of its transformative power and enduring impact on my practice and patients alike. Beyond its aesthetic benefits, Silhouette Soft embodies a philosophy of artistry and innovation that continues to inspire my pursuit of beauty enhancement. In the ever-evolving landscape of aesthetic medicine, Silhouette Soft stands as a testament to the convergence of science and artistry, offering a harmonious blend of immediate results and long-term rejuvenation. As aesthetic doctors, it is our privilege and responsibility to embrace and champion such advancements, empowering our patients to embrace their beauty with confidence and grace.

While dermal fillers and PDO (Polydioxanone) threads are valuable tools in the aesthetic toolkit, Silhouette Threads offer unique advantages that distinguish them in certain scenarios. Dermal fillers excel in restoring volume and softening wrinkles, making them ideal for addressing static lines and hollow areas of the face. However, they are limited in their ability to provide significant lifting effects, particularly in cases of moderate to severe skin laxity. On the other hand, PDO threads offer a minimally invasive solution for lifting and tightening sagging skin. They are

O ver the past decade, the landscape of aesthetic medicine has witnessed remarkable advancements, particularly in the realm of minimally invasive procedures. Among these innovations, Silhouette Soft thread lift has emerged as a beacon of transformative enhancement. As an aesthetic doctor, I have had the privilege of incorporating Silhouette Soft thread lift into my practice since its inception in 2012. In this article, I will share my journey with Silhouette Soft, elucidating its benefits, comparative advantages over polydioxanonethreads, and the profound impact it has had on my patients' satisfaction and confidence. Dr Awfa Paulina MBchB MRCGP of Clinica Medica

introduction of 12 short threads has made it possible to achieve better results. In the recent years also combining Silhouette Soft with dermal filler and neuromodulators in the depressor muscle will help with enhancing the lift. One of the standout benefits is its versatility, allowing me to address various aesthetic concerns ranging from sagging skin to facial contours with precision and efficacy. The minimally invasive nature of the procedure appeals to patients seeking subtle yet significant enhancements without the downtime associated with traditional surgical facelifts. Furthermore, Silhouette Soft offers a tailored approach, enabling me to customize treatment plans according to each patient's unique anatomical features and aesthetic goals. Whether it's lifting the midface, defining the jawline, or rejuvenating the neck, these threads provide unparalleled flexibility and control, ensuring natural-looking results that harmonize with the individual's facial architecture. Moreover, the biocompatible nature of Silhouette Soft minimizes the risk of adverse reactions, ensuring a safe 1 and comfortable experience for my patients. The convenience of the procedure, often completed within an hour with minimal discomfort, further enhances its appeal, particularly for individuals with busy lifestyles.

WHY I PURSUED SILHOUETTE SOFT:

In 2011 I travelled to Barcelona to learn and train in Silhouette Soft threads. My motivation at the start was hoping to find a solution for rejuvenating lower face because I didn't have a solution which would work for my patients beyond a dermal filler. But soon it became obvious that Silhouette soft is more than just a lower face solution, they enable me to sculpt facial contours with unparalleled accuracy. Silhouette Soft consisting of resorbable suture with moulded cones made of a resorbable material, which allows the control and flexibility needed to address all aesthetic concerns—from sagging skin to facial asymmetries and let's not forget dull skin. This level of control ensures that each treatment is tailored to the individual's unique features and aesthetic objectives, resulting in natural-looking outcomes that harmonize with their facial features.

with no downtime, it’s painless and doesn’t carry the associated risks that invasive treatments can. And whilst it’s great as a stand alone treatment, it’s also extremely effective when used in conjunction with other treatments such as fillers. Non-surgical treatments are growing and the need for combination treatment with injectables and other EBD devices is important to achieve desirable results to discard surgical procedures. For all of these reasons it’s no surprise that it appeals to younger patients - both male and female - who are keen to undergo non-invasive aesthetic procedures in order to improve the skin’s function and maintain good skin health and appearance. Women aged 30 to 65 make up most of all skin rejuvenation patients. This demographic will further develop as more and more young women seek treatments that will delay the signs of ageing, and older women increasingly seek procedures that will maintain results they have achieved with other treatments. Millennials and Generation Zs continue to flock to aesthetic practices and medspas in significant numbers. These new patients are open to preventative procedures, including energy-based face and body modalities.

AN ROI EXAMPLE BASED ON 2 TX PER DAY: £150 PER TX

= £300 per day = £1500 per week

= £6000 per month = £72,000 per year Increase this by 1 Tx per day and revenue potential increases to £108,000 per year.

I see first-hand in my clinics in and facial rejuvenation is now, when in the right hands, more effective than ever. So it’s no surprise that the demand has never been greater as patients seek to benefit from treatments that offer effective, natural- looking results without the downtime that some alternative treatments pose. I think that this popularity is linked to the trend-shift that we are seeing both nationally and globally for ways to enhance and improve the face and body whilst maintaining a natural look. With the media awash with scare stories of ‘botched’ surgical procedures (both abroad and in the UK), many people are looking for safe, risk-free ways to enhance their body and looks, which is where radiofrequency is key. London’s Harley Street and Truro that non-invasive body contouring

displacement of retained interstitial fluids which manifests itself with the removal of excess fluids in subcutaneous adipose tissue and the reduction of adipose tissue volume. Its benefits are multifaceted too. I’ve seen fantastic results using radiofrequency on lax skin on the neck, face, tummy and other areas of the body. It helps to firm skin, leaving it tighter and rejuvenated.

Radiofrequency offers treatment options for multiple skin concerns, from the impacts of ageing which include wrinkles and sagging skin, to cellulite and fat reduction, as well as improving the texture of skin. I use Sculpt and Shape in my clinics in London’s Harley Street and also Truro and have seen, and continue to see, great results on lax skin not only on the face and tummy both also on other areas such as the neck and arms. Whilst it’s highly effective as a standalone treatment, a huge draw is the fact it can also be used in conjunction with other treatments such as dermal fillers to create a personalised and results-driven treatment programme. Its versatility allows for combination treatment approaches with the huge draw being that there is no downtime or lifestyle restrictions like alternative treatments. RF also comes with low consumable costs and fast treatment times, making it an attractive ROR in a short period. Finally, the fact that it doesn’t come with the pain and risks that some invasive treatments do means that it’s an attractive option in terms of safety and comfort too.

SCIENTIFIC BACKGROUND

- Different types of RF technologies and their biological effect RF technology comes in a number of different categories. • Monopolar and Bipolar, the former which creates fibroblast stimulation (39-24 °C), and the latter which causes Hyperthermia/Apoptosis (43-50 °C). These cause skin tightening, rejuvenation, cellulite reduction and fat reduction. • Unopolar, Multipolar and Fractional (microtips or microneedle), which cause cell damage by denaturation and coagulation (50-80 °C), and ablation (100-120 °C). The effects of these are resurfacing. UNDERSTANDING THE MARKET - WHY RADIOFREQUENCY? The radiofrequency-based aesthetic devices report highlights that the market was valued at $1,052.6 million in 2020 and is expected to reach $3,204.1 million by the end of 2030. The market is expected to grow at a CAGR of 12.11% during the forecast period from 2021 to 2030.

WHY RADIOFREQUENCY?

INTRODUCING RF TECHNOLOGY

"A huge appeal of radiofrequency to many of my patients is the fact that it’s non-invasive so it comes with no downtime, it’s painless and doesn’t carry the associated risks that invasive treatments can."

In order to appreciate the power of non-invasive body contouring and facial rejuvenation through radiofrequency, it’s first important to understand what exactly radio frequency is. Radiofrequency, or RF, is the portion of the electromagnetic spectrum that lies between 3kHz and around 300GHz. Radiofrequency causes an oscillation of molecules which heats tissues.

There are a number of important considerations to be taken into account including: • Selecting the right device for your practice • How do I know it’s safe? • Training and certification • Ongoing support • Social media • Introductory pricing • Package/loyalty programmes • Revenue Potential - Common concerns COMMON CONCERNS ABOUT REVENUE POTENTIAL INCLUDE: • Expensive outlay • Concerns about making enough profit • What is a realistic ROI? • How many patients do I need to treat?

HOW RADIOFREQUENCY IMPACTS THE BODY

Radiofrequency impacts the body in a number of ways. It results in vasodilation, or increased blood flow. The impact of this on the body is increased oxygen level and increased metabolism. It also causes a contraction of collagen fibres, and neocollagenesis - fibroblast stimulation and formation of new proteins of collagen and elastin fibres. Radiofrequency also causes the

My patient refrained from going on holidays due to lack of confidence in her appearance

My patient was fit and healthy, with no known contraindications for the Lanluma procedure. We developed a positive patient-health care professional relationship over the years and patient trusted me to explore sustainable, longevity options for her arms’ skin elasticity to help her to regain confidence for better quality of life. As for medical professional, the reason for Lanluma selection for me was the Poly-L-Lactic Acid safety profile 3, 6 and its principal ingredient, provides results that appear gradually and last more than two years 7 . Backed with the support from Sinclair, with comprehensive protocols and video tutorials on the application of the product being available in Sinclair College 1, 2, 4 . The consistent results are offered over course of treatments without over- volumising soft tissue with uniform results without palpating edges of placed product.

On the day of the treatment, a summary of initial consultation was provided with answering patient’s questions, aftercare written instructions including the importance of massaging and consent forms. The baseline photos were taken as a part of assessment and monitoring progress with each session.

BACKGROUND

TREATMENT PLAN

"I felt my patient was a perfect candidate for Lanluma."

Prior to enquiring about treatment in arms’ region, the patient has been visiting practice for approximately 5 years to address concerns such as androgenetic alopecia, facial volume loss, muscle-relaxing injections for upper face and skin photodamage. An initial contact was via our website, following Google search for Platelet Rich Plasma (PRP) locally. Delighted with results, the patient wished to explore options for skin laxity of the body. Despite active regime, regular exercises and normal BMI index, she felt that skin around the arms has become a big concern during menopause. The patient tried treatments including skin boosters and bio remodelling therapies with hyaluronic acid around 2 years ago, however she felt results were not satisfactory and not long lasting enough to maintain treatments. She stopped going on holidays in warm weather and felt the choice of her clothes was limited with skin coverage above elbows.

The patient was offered a consultation as primary stage in developing individualised treatment plan. Whilst there were no medical contraindications for the treatment with Lanluma, it was important to establish several treatments would be required to achieve gradual results. The patient’s compliance with after care, especially massaging, was a key to minimise possible side effects such as nodules and offer uniform results in treated areas. I suggested 3 treatments with 4-6 weeks gap between sessions. Cooling-off period was offered with comprehensive follow-up encrypted email, covering information about Lanluma, treatment plan, possible side effects and after care to enable patient to give informed consent.

RESULTS AND PATIENT’S FEEDBACK

BEFORE / AFTER Three Lanluma V in total across three sessions. Before and after photos taken 24 weeks apart.

Results and photos were compared every session with final results evaluation 4.5 months after treatment initiation that included three sessions every 4 weeks. The patient rated an overall experience and results as “very satisfied’. She was pleased with longevity of results that Lanluma has to offer and was advised to attend annual review to observe results with potential maintenance session to allow long benefit results of the treatment.

What’s your background in aesthetics been?

17 years ago, I started in Sales and have worked with various hyaluronic acid brands from dermal fillers to neuromodulators and more! I have been involved with product launches, business development and clinical training. I also spent 3 years in surgical, which gave me a great insight into this side of the aesthetic business. I have a real passion for this sector and love how fast paced and dynamic the industry is and how quickly Sinclair can adapt. What are you most excited about this year? Supporting our partners! Patients want a more personalised approach and are seeking more bespoke treatment plans. As a result, combination treatments are increasing in popularity. Sinclair are delighted to offer a product portfolio through which all needs are met i.e. innovation in HA with MaiLi, regenerative treatments with Ellansé and collagen stimulating threads Silhouette Soft.

The body market is growing and the newest exciting innovation in our portfolio is collagen stimulating filler Lanluma, winner of the prestigious Best Injectable Body Filler award at AMWC. We also launched our energy-based device division into the UK, which complements our injectable range. fantastic, experienced, and enthusiastic team at Sinclair as well as working with our partners at prestigious "I am excited to be working with a medical bodies such as the BACN, JAN and training providers such as Cosmetic Courses."

What do you think are some of the big changes or challenges happening in the industry? Lack of regulation and non-medics performing aesthetic procedures. This causes complications which in turn generates negative media coverage of aesthetics and the industry. I feel there is poor public awareness about the dangers and the importance to go to a medical practitioner. However, the industry is taking steps towards regulation and consistently driving education to the patient.
