Due to the in-network deductible change for the HSA I plan, current and new enrollees in that plan will receive a new ID Card. There will be no new ID Cards for the two other medical plans unless you are a new enrollee. No physical ID Cards will be sent for the new Dental and Vision plans. Members can print and/or download ID Cards to their smartphone via www.myuhc.com for Medical, Dental, and Vision plans. For more information on your plans, please call UnitedHealthcare:

An HSA bank account needs to be established for HSA participants and their applicable contribution amount elected in ADP. You can update your beneficiary information at any time year-round through ADP for life and accidental death and dismemberment. You must go to T. Rowe Price to update your beneficiary information for your 401K.
