ELIGIBILITY
Option 1:
Provides coverage beyond group long- term disability (LTD) coverage to give you more income replacement Employees may purchase additional coverage to a maximum of $9,000/month or 65% income replacement, whichever is less
• 60% income replacement* after 6 months • Maximum monthly $4000/month • 65% income replacement* after 4 months • Maximum benefit $4000/month
Option 2:
No medical exams Disability defined as “Specialty/Subspeciality”
Option 3: (available to employees earning more than $80,000 in annual UC base pay) • 60% income replacement* after 6 months • Maximum benefit $20,000/month • OR – • 65% income replacement* after 4 months • Maximum benefit $20,000/month Disability defined as “Any Occupation” Guaranteed issue
Individual ownership - this is not "group" coverage; you will own your policy and will have the ability to take it with you should you leave UC Physicians
