10. What role do Retention Alerts play in meeting HCC student retention goals? Retention Alerts are vital. Without these reports, CARE team members are often unaware of students experiencing personal challenges that may put a student at increased risk of college departure or drop-out. Student Reporting Forms The following online forms are conveniently located on the HCC website for completion when needed. To access the forms simply go to HCC’s webpage, click the Faculty & Staff tab and then scroll down to the Popular Links green box in the lower right, then click on Student Report Forms. Additionally, the forms can be found at https://www.hagerstowncc.edu/student-reporting- forms. The General Student Report Form is used to report students who appear to exhibit unusual behaviors of concern, or whose on campus behaviors may have violated the Code of Student Conduct. Completion of this form allows community members to share a report with the Dean of Students’ Office to determine the most appropriate response; reporters may report for informational purposes only. The goal is to intervene before these students develop more serious problems such as mental health concerns, failing out of classes due to extreme personal challenges, or exhibiting disruptive behaviors that go against the campus community standards. The report goes directly to the Office of the Dean of Students for response within 48 hours. For use by students, faculty, or staff . HCC's Dean of Students serves as Title IX Coordinator for Students (to address sex-based discrimination) and 504/ADA Coordinator (to ensure compliance with the Americans with Disabilities Act, Sections 503 and 504 of the Rehabilitation Act of 1973, and other federal and state laws and regulations pertaining to persons with disabilities). Students should submit this form for matters regarding Title IX or 504/ADA. The Retention Alert Form is used to share information about students who are struggling with personal challenges such as frequently missed classes, food or housing struggles, or possible mental health challenges. The form is located in Self-Service. Navigate to the Retention Alert tab. Search for a student using their student ID number, and choose the Contribute Retention Info tab. Submitting this form will connect the student to the HCC CARE Team, a cross-campus group of professionals who assist students by offering support and resources when a student faces financial, health and well-being, or other issues that interfere with their ability to thrive academically and personally. Faculty members, please share your concerns about a student’s well-being or overall success by submitting a Retention Alert. Please don’t assume someone else has done so. The team will consolidate information. It is better to have duplicates than no report at all. For completion by faculty or staff. The Academic Integrity Reporting Form is for making reports related to academic dishonesty such as cheating, plagiarism, or misuse of College computers. When these rules appear to have been violated by a student, the faculty member’s report is sent to the Vice President of Academic Affairs and Student Services who will either send a letter of reprimand to the student or schedule a meeting of the College Hearing Board. The Academic Integrity Reporting Form goes directly to the Office of the VPAASS for processing. For completion by faculty or staff .
