As we share this new edition of FractureFocus, it is encouraging to see the continued growth of the global Capture the Fracture® network, now with more than 1,200 Fracture Liaison Services (FLS) worldwide. This progress reflects a strong collective commitment to improving secondary fracture prevention. Capture the Fracture ® is now entering a new phase, guided by the “Road to Gold” vision. While the expansion of FLS has been significant, fewer than 13% have achieved Gold Standard. Our priority is therefore clear: to support services in advancing the quality, impact, and sustainability of care. Through practical tools, shared expertise, and scientific collaboration, we aim to help every FLS reach higher standards and deliver better patient outcomes.
This issue highlights both progress and opportunity. New evidence from Korea demonstrates that coordinated FLS care can reduce mortality and refractures while improving efficiency. National initiatives in Brazil and the UK demonstrate how multidisciplinary collaboration and advocacy work are driving meaningful change at the local level. Looking ahead, the WCO- IOF-ESCEO Congress in Prague will provide a key platform to exchange knowledge and accelerate progress. Closing the fracture care gap remains an urgent priority. By strengthening systems, aligning care with patient risk, and working together as a global community, we can deliver better care for all.

CAPTURE THE FRACTURE ® “ROAD TO GOLD” VISION IMPROVEMENT AND SUSTAINABILITY IN FLS
Secondary fracture prevention is entering a new phase: one focused on quality, impact, and sustainability. Through Capture the Fracture ® ’s (CTF) “Road to Gold,” the International Osteoporosis Foundation invites healthcare providers to strengthen their Fracture Liaison Services (FLS) and strive for Gold-Standard care. Previously, CTF prioritised promoting and supporting the development of new Fracture Liaison Services (FLS) worldwide. However, despite the significant growth of over 1200 FLS in the network, fewer than 13% have achieved the Gold Standard.

For this reason, the next phase aims to focus on developing solutions that support FLS in their journey towards higher standards of care and long- term sustainability. These include organising events that encourage Best Practice sharing, developing guides to support FLS management, supporting real-world evidence through scientific initiatives and much more. Additionally, there is a clear need to further educate FLS on systematic fracture risk stratification. By aligning treatment pathways with patient risk profiles, FLS can ensure high-risk patients receive timely, intensive care while also improving adherence rates.

New Study
FLS SAVES LIVES AND PREVENTS REFRACTURES KOREAN STUDY SHOWS
A new Korean study highlights the life-saving impact of Fracture Liaison Services (FLS) in the management of hip fracture patients. Conducted at the FLS of the Daejeon Eulji Medical Center, the first FLS established in Korea, the prospective cohort study compared outcomes before and after the implementation of a coordinator- based FLS.
The implementation of a coordinator-based FLS has allowed us to deliver more timely and holistic care for elderly hip fracture patients—improving survival, reducing refractures, and ensuring continuity beyond the acute hospital stay.

06 || Korea: FLS saves lives and prevents refractures
The findings demonstrate strong clinical benefits. Patients managed through FLS experienced faster access to surgery and shorter hospital stays, reflecting improved coordination of care. Most notably, mortality rates were significantly reduced, both during hospitalization and at one year, while refracture rates were nearly halved. The study also showed a substantial increase in osteoporosis treatment initiation and better nutritional management, with more patients receiving calcium and vitamin D supplementation and improved albumin levels over time. High levels of patient and caregiver satisfaction further underscore the value of this coordinated care approach.
While functional recovery outcomes remained similar within the first year, the results strongly support FLS as an effective model for improving survival and preventing secondary fractures. As populations age, expanding FLS programmes could play a crucial role in enhancing long-term patient outcomes.

CLOSING THE FRACTURE CARE GAP THE ROLE OF THE ROYAL OSTEOPOROSIS SOCIETY IN THE UNITED KINGDOM
An interview with Craig Jones, Royal Osteoporosis Society CEO
commitment has driven nationwide implementation and measurable improvements in patient outcomes. However, significant disparities remain, particularly in England, where only about half of NHS Trusts currently provide FLS, leaving many patients without adequate post- fracture care.
In our latest interview, CEO Craig Jones shares a candid view of the progress being made in the UK —and the work still to be done. Since 2022, the Royal Osteoporosis Society has accelerated efforts to close the fracture care gap across the UK, with notable progress in expanding Fracture Liaison Services (FLS). Wales stands out as a leading example, where strong political

He also outlines the practical priorities for FLS teams today, from improving patient identification and treatment initiation to strengthening long-term adherence through better follow-up and digital tools. He notes that achieving consistent, high-quality care across the UK will require more than local effort. Stronger national leadership, sustained investment, and coordinated advocacy are essential to ensure no patient falls through the cracks.
For FLS to consistently deliver the quality of care we want to see, they need the resources and capacity to support people through the whole pathway—not just the early steps. Some services can only do parts of the model, and that means patients at high risk can fall through the cracks or drop off treatment.

NEW TOOLKIT ON THE IMPACT OF FLS IN BRAZIL: AN IOF INITIATIVE TO STRENGTHEN FRACTURE PREVENTION
The IOF, with support from Amgen Brazil, has launched a toolkit to highlight the clinical, economic, and social value of Fracture Liaison Services (FLS) in Brazil. Central to the initiative is the report “O Impacto Econômico, clínico e no paciente da FLS: Um relatório baseado em histórias de sucesso em três países da América Latina”*. The report aims at raising awareness among healthcare managers, guiding hospital decision-makers, and encouraging adoption of coordinated post-fracture care.
It highlights the burden of fragility fractures in Brazil and demonstrates that FLS improve outcomes for patients with fragility fractures by reducing morbidity and mortality, while strengthening health systems through systematic risk assessment and prevention. In recent years, Czech clinicians and healthcare leaders have demonstrated strong dedication to advancing secondary fracture prevention. The six mapped FLS collectively identify more than 3,000 patients each year, ensuring that individuals who experience a fragility fracture are assessed and guided toward appropriate osteoporosis management. The success of FLS in the Czech Republic reflects the dedication of multidisciplinary teams, including orthopaedic surgeons, endocrinologists, rheumatologists, nurses, and coordinators working together to transform fractures into opportunities for prevention.
As Prague prepares to welcome experts from around the world, the Czech FLS community stands as an inspiring example of how collaboration and clinical leadership can translate global recommendations into meaningful local impact.
