If it has been a relatively long time (5 or more years) since you had a rabies booster vaccination or a titer determination, you may elect to have a booster vaccination instead of having a titer run. Cost of the vaccination will be $300. Sign up for either a titer or rabies booster as you register. AMERICANS WITH DISABILITIES ACT All meeting rooms are accessible via wheelchair in the Convention Center. Contact SWVS at 972-664-9800 no later than August 31 for assistance with any special needs so that we Press and media must register with SWVS prior to attending the Symposium. Press credentials will be issued to authorized media. GUEST REGISTRATION All guests include family members or guests who are not veterinarians, veterinary technicians, o¤ce sta£ or practice managers. This allows guests to attend non-CE events, exhibits, and non-ticketed events. Guests are welcome to attend ticketed events (social only) with the purchase of a ticket for those events. Guests who wish to attend CE sessions must register and pay. CHILDREN Children age 17 and under do not require a name badge but must be accompanied by a registered attendee. A ticket is required for anyone over the age of 2 years at all ticketed social events. Children are not permitted in any CE sessions. PROCEEDINGS 2018 Proceedings will again be available online through Veterinary Information Network. All registered attendees will have access to the conference notes online. There will be no books or flash drives available. PARTNER POWER PACKAGE We have made registration for the Partner Power Package (which includes one DVM plus three technicians and/or o¤ce personnel) easy so that all included in this package may register at once with a single form of payment. The DVM will complete his/her registration with a single form of payment and then be instructed to add the three additional attendees at one time to complete the Partner Power Package (POP). ON-SITE REGISTRATION Register by August 31 and bring your email confirmation to the Registration Counter, located in Hall 4 West Registration of the Henry B. FORMS OF PAYMENT VISA, MasterCard, American Express and checks are accepted for payment. All checks must be made payable to “SWVS” and must be drawn on a U.S. bank. If you want to register online and pay by check, a payment option for “check” is available, and your registration will only be considered confirmed upon receipt of the mailed check. IMPORTANT NOTE: Checks must be postmarked by the deadline for discount pricing. Any payment received after the discount registration deadline will be assessed the registration fee applicable to the date the check is received. Checks must be received within 5 business days after registering online. NSF CHECKS AND DECLINED CREDIT CARDS In the event that checks are returned as NSF or credit cards are declined, you will be notified immediately. A $35 NSF fee will apply and a replacement check will be required. Payment issues must be resolved within 5 business days of being notified, or your registration will be invalid. Payment must be received by each respective deadline to qualify for the reduced registration fee. REGISTRATION CONFIRMATIONS Registration confirmations are sent by email, so please provide your email address! If you do not have an email address, you will receive a confirmation within 2 weeks in the mail. If you are considering any Interactive Labs/Master Classes/CE Tours/Meet the Experts Lunch, space is limited; you may want to go online to register. Mailed or faxed forms require manual processing and there is a chance you may not receive your first selections. If you do not receive confirmation within 2 weeks, please call 972-664-9800. PRIVACY NOTE: SWVS WILL NOT sell your personal information to a third party. We use all information to accommodate your interest in our products and services. CANCELLATION POLICY Cancellation of registration and/or social events must bemade inwriting via email to info@swvs.org, fax 972-850-7469, or postmarked by August 31. Cancellations cannot be taken over the phone. A $50 handling fee will be charged to cover administrative costs.

VETERINARY STUDENTS & TECHNICIAN STUDENTS REGISTRATION

Veterinary and technician students are required to provide acceptable identification when registering for SWVS 2018. Acceptable documents are: a copy of a current student identification card, a letter of verification, or a letter written on o¤cial letterhead signed by a professor. Back Again This Year! Students do not have to wait for a confirmed registration when registering online with acceptable proof of identification. When registering online, students can upload their documentation and proof of Student I.D. verification that is required. This will confirm the student registration immediately, and the student will receive an acknowledgment of his/ her registration. Students who choose to fax or mail their registrations must attach a copy of the acceptable proof of identification document with the registration. DESIGN CONFERENCE The annual Design Conference will be held this year on Thursday and Friday at the Henry B. González Convention Center. Attendees have the option to attend one or both days of the Design Conference. Proceedings are provided to registrants only. See page 14 for complete details. Registration to SWVS is not required to attend the Design Conference. You must be registered for SWVS 2018 (complete or one-day) to attend these ticketed CE o£erings, with the exception of the Design Conference. An additional fee is required. Registrations are accepted on a first-come, first-served basis until the maximum capacity is reached. IMPORTANT NOTE: The registration deadline for these ticketed CE o£erings is August 9. A $25 cancellation fee applies for cancellations made prior to August 9. No refunds of ticketed CE events will be given after August 9 for cancellations. Cancellations cannot be taken over the phone. These ticketed CE o£erings require a minimum number of registrants. If a session has less than the minimum number required, it will be cancelled in late August. Participants will be notified and appropriate refunds will be processed. TITERS DETERMINATION AND RABIES VACCINATION CLINIC SWVS o£ers you the opportunity to have blood drawn for a titer determination. Blood will be drawn on Saturday, packaged, and shipped to a laboratory for titer determination. Results will be sent to the SWVS sta£, who will forward your titer to you.
