The Special and Community Support Division provides information regarding available state and county social service programs. An emergency food pantry and medical equipment loan-out is available for Sunrise residents who may need temporary assistance due to loss of employment or other issues. Five Wishes lecture is for anyone 18 years or older, married or single, parents, adult children and friends. More than 19 million people have used it. Because it works so well, lawyers, doctors, hospitals, hospices, employers and retiree groups are handing out this document. This document meets the state requirements under the law. A professional from Continuum Care Health will be here to go over how to fill it out. You will be able to leave the lecture with peace of mind. The City of Sunrise operates an Emergency Food Pantry with canned goods for Sunrise residents in need. Visits to the Pantry are limited to no more than once every three months until the crisis has passed or an alternate agency can fulfill the need. It is by appointment only and must be arranged with the Social Worker. For information, call (954) 746-3670 between 8:30 am and 4:00 pm.

Prescription Discount Card Program Sunrise residents who are without health insurance, a traditional pharmacy benefit plan, or have prescriptions not covered by insurance can take advantage of this plan. The savings is approximately 20%, with no income or age requirements. Please call the Sunrise Senior Center at (954) 746-3670 or come by the Senior Center for information. Bring a Florida Driver’s License; a Broward voter registration card; Social Security numbers of all owners; and a copy of the recorded deed or tax bill. Resident aliens must bring a permanent resident green card or proof of asylum. Learn about how you can reduce the risk of falls, improve your health and reduce the risk of healthcare expenditure. A Walgreens Pharmacist will be on site to review your medications, so bring a list of the medicine you presently take, including the milligram amount and frequency.
