A NOTE FROM SYLVIA As the shortest month of the year kicks off and we gradually gain daylight, I make sure to find time to enjoy these sunny, wintery days. I love how Minnesotans find unique ways to brave the chill. From cross-country skiing to ice fishing to a favorite of mine, cozying up with a book by the fireplace, we find ways to thrive during the cold weather. As we begin our subtle thaw into spring, I am excited to share some wonderful programming available on-air, on-demand and online on Twin Cities PBS. This month, we’re premiering a new Minnesota Experience documentary, Film in the Cities: Radical Roots of Youth Media . The film explores the story of a group of young artist-teachers who launched a groundbreaking arts education program centered on filmmaking here in the Twin Cities. At its core, the story explores the unadulterated joy of burgeoning young filmmakers in the 1970s. Watch the premiere on Monday, February 21 at 8 p.m. on TPT 2 or stream online at tpt.org/minnesotaexperience . In last month’s magazine, we highlighted a new web series that is part of TPT’s Racism Unveiled initiative – That Got Weird . Hospital at Home: The Future of Health Care in Your Living Room By Beth Baker
Jim Petersen, 83, who lives alone in an assisted living community outside Denver, recently had pneumonia. But rather than be admitted to the hospital, he was offered another choice: hospital-level treatment in his home.
In-home visits "invert the relationship" between doctor and patient, he adds. In a hospital, the patient wears a gown and lies in a bed while doctors do rounds. In contrast, Diaz is invited to enter the patient's home. During an appointment with Petersen, the two discovered they both loved fishing and hunting and shared roots in northern Illinois. "It makes the ability to connect with a person that much easier, and it's important clinically, determining a plan of care that will work in that context."

Petersen didn't hesitate. "In plain English, there's no place like home," he says. "You can get good care in a hospital, but you never sleep really good." In the wake of the COVID-19 pandemic, a growing number of older patients are more eager than ever to avoid hospitalization. Depending on their condition, many can receive the full complement of professional services in their home, paid for by Medicare as if they were in the hospital. In Petersen's case, nurses and physician assistants came to his home to monitor him and administer antibiotics. "With the technology today, they can take x-rays and EKGs and blood work right here in my apartment," he says. "They were just great. I stayed here and recovered." His physician, Dr. Manny Diaz, used to work in a hospital, but is now medical director with Advanced Care-Denver. "I believe [this] will be the future of health care, moving as much as we can effectively and safely into people's homes to provide comfort at a lower cost," Diaz says.
Not all patients are eligible for home care, such as those experiencing chest pain, stroke symptoms, severe abdominal pain or drug overdoses. Dozens of other common conditions, though, including congestive heart failure and urinary tract infections, can be treated at home. As technologies advance and as people age who are comfortable with computers and cell phones, home-based delivery of medical care is likely to grow, especially if CMS (Centers for Medicare and Medicaid Services) continues to fund acute care at home, say practitioners.

Are These the Little Girls I Carried? By Lyla Blake Ward
Last year, when my younger daughter Belinda turned 65, I suddenly realized: we are all seniors. Amazing and wonderful — but weird, right? Of course, this didn't just happen. I've known for a long time she and her sister Gina, 68, were growing up. I couldn't help but notice Gina, teaching her grandchildren to ride their bikes, is the spitting image of the girl I taught to pedal her first tricycle. Or that Belinda, with the shelf full of Emmy Awards for her work in children's television, bears an uncanny resemblance to that young girl who won a prize for an essay she wrote in fifth grade. If you find yourself, unbelievably, the mother of could-be retirees, your first instinct may be to wring your hands and wail: Where has the time gone? Since I am one of the lucky ones whose memory is intact, I don't have any trouble recalling times gone by; my problem, in relation to my daughters, is reminding myself, at this point, they know as much (well almost as much) about life as I do. I did use our times together when they were children — maybe 9 or 10 — to let them know how I saw things. Very often, one or the other or both would hang out in the kitchen with me as I prepared dinner and advanced my philosophy on money, marriage, and feminism: equal rights forever! A good part of what my daughters know, and I do not, is the result of having been in the business world. They have dealt with sometimes difficult bosses or decision makers, negotiated contracts, and met new associates as they traveled wherever the job took them.

I have to recognize that my "girls" are now women. They have had children, run households and had long successful careers. I have complete confidence in their ability to handle the most delicate situations. Yet, every so often, when one of these undisputedly mature women is helping me load the dishwasher after dinner, I have the uncontrollable urge to say: Don't forget — anything plastic goes on the top shelf.

How Can Medical Schools Fix Systemic Issues in Health Care?

Minnesota consistently ranks as one of the healthiest states in the nation but as the conversation around racial disparities and social inequities has risen to the forefront, research shows that Minnesota's portrayal as the home to healthy residents only paints half the picture. Census data shows that Minnesota's population is more than 80% white and just 7% Black. Despite this vast difference in population size, Minnesota's Black community is overrepresented in chronic health issues when compared to their white counterparts. Black men in Minnesota have a higher mortality rate from cardiovascular disease than white men. Black women die of breast cancer at a 24% higher rate than white women, although their incidence rates - how frequent a disease occurs in a population - are 22% lower. The difference in health outcomes between white Minnesotans and Minnesotans of color is so pervasive that it can be pinned down to a zip code. These racial disparities in health are not unique to Minnesota, and are indicative of a systemic issue that is plaguing the nation. To address these inequities, university students and faculty at several medical schools have called on their administrations to create programs that address racial inequities that have caused disparities in health, pointing to inequality in care, racial bias among physicians and the lack of representation found in their study material. Making a Change in How Race is Addressed in the Classroom Constructing a racially and culturally inclusive curriculum has become a focal point for medical
schools across the country as the classroom serves as the introduction to medicine for students. What students learn in their first few courses in medical school may influence their practice for years. To change their approach to educating future physicians, the University of Minnesota Medical School recruited Dr. Ana Núñez to head their inaugural office of Diversity, Equity, and Inclusion (DEI). Dr. Núñez serves as a Vice Dean in the medical school and was brought on after last summer's civil unrest sparked by the Minneapolis police murder of George Floyd. The medical school's curriculum has been a focal point in making institutional changes. Knowing this, administrators have brought medical students in on the process to give their input as they work to reconstruct their education. "As we talk about curriculum, what really matters is: What are the student's experiences when they come to us," Dr. Núñez said. "Because that's how you get it right in terms of that engagement piece, because people sitting in a little conference saying, 'This is how we're going to change stuff,' without the voice of the lived folks just never get it right." Medical students like Sally Jeon and Tegan Carr have worked to fix the medical school's approach to education. Both have served as their class DEI officers and are a part of the UMN Medical Education Reform Student Coalition (MERSC) that formed as a response to the police murder of George Floyd.

sending a letter that listed what changes they'd like to see in response to the nation's racial awakening. Those changes include emphasizing an equity- based curriculum, establishing plans for faculty development in health equity instruction, removing race as a biological indicator in medicine, diversifying their standard patients and
strengthening ties with communities of color.
Though medical students want to be a part of this change, their studies and time reserved for clinical work off campus limits how much they can do. "For me as a first-year coming in, I had a sense that maintaining sort of the momentum of this could be a challenge," UMN medical student Sally Jeon said. "While it is absolutely crucial to have student voices centered through this experience, students cannot do this alone."
Indian and Minority Health, for doing the work from within the institution that goes beyond school hours. Much of DEI's work in health care or other industries has been seen as a sort of caveat or add-on to the work, but Dr. Núñez believes that it should be integral to how medicine is taught going forward.
"There is no area that is exempt from health disparities," Dr. Núñez said. "Not a one."
