The purpose of this form is to assist MAA in determining whether, or to what extent, a reasonable accommodation is required for an employee with a disability to perform one or more essential functions of their job. This form will be treated confidentially and maintained separately from the employee’s personnel file. A detailed explanation of the rights and obligations of employees under the ADA is contained in The Americans with Disabilities Act: Your Employment Rights as an Individual with a Disability , which is available from your State ADA Coordinator’s Office in the Department of Labor and Workforce Development. You may also contact MAA’s Benefits department for more information. In order to complete this form, you will need to discuss the essential functions of your job with your supervisor.
