Full-time associates who have completed 30 days of service with Averitt are eligible to participate in our health benefits.
ELIGIBLE DEPENDENTS AND THE DOCUMENTS YOU NEED DEPENDENT DOCUMENTS FOR VERIFICATION
ENROLLING IN BENEFITS As a new Averitt associate, your benefits take effect after 30 days of service. Please ensure you complete your enrollment within the first 30 days of your employment. The choices you make now are final until the next annual enrollment period. After your initial enrollment, you can only make changes to your benefits if you experience a qualifying life event, such as getting married or having a child, or during each year’s annual enrollment.

QUALIFYING LIFE EVENT If you experience a qualifying life event, such as a birth or marriage, during the plan year, you must send documentation to the Benefits Team within 31 days of the event. To do so, fax dependent verification documents to Averitt Benefits Administration at 931-520-5101 or email averittbenefits@averitt.com .

Children will age out of benefits at age 26 and will be automatically removed at the end of the month in which they turn 26.
HOW TO ENROLL:
Visit InsideAveritt.com/MyPortal and follow the links to log in to MyPortal. Log in using your email address and your MyPortal password . • To enroll in medical, dental, vision, and FSA or HSA, click the “Qualifying Event” widget on your dashboard, then double click on “New Hire” to begin the enrollment process. Follow the prompts to review your tobacco use status, choose your benefits, and then submit your selections. Call the Averitt Benefits Team at 800-233-9944, option 3, if you need help enrolling or have questions about benefits. • If you’re adding any dependents, submit required documents to averittbenefits@averitt.com or fax them to the Averitt Benefits Team at 931-520-5101 with a cover sheet that lists your name, phone number and the last four digits of your Social Security number.
