The programme supports you to develop skills which will help you to live well with one or more long-term health conditions, such as but not limited to: COPD, Asthma, Diabetes, Stroke, Heart Conditions, Multiple Sclerosis (MS), Parkinson’s, Arthritis, Cancer, Crohn’s disease, Chronic Pain, Fibromyalgia and Depression. The programme has been proven to be effective in: • Enhancing self-efficacy • Improving health behaviours • Improving social engagement • Reducing feeling of depression and anxiety Some more programme details: • It is a scripted programme • It’s delivered in group of 8-12 participants • It runs 2.5 hours a week for 6 weeks • It’s delivered by 2 trained Leaders, one of whom has to be living with a long-term health condition
