This will allow businesses who need to report their funds to the Division as well as submitting holder reimbursement forms and documents. We accept .csv, .txt, .hde, .hrs, .rpt and .pdf files. Please note, be careful with the file names because special characters cannot be part of the name of the file you are uploading. Payment can be submitted via check payable to Unclaimed Funds and sent to 77 S. High St, 20 th floor, Columbus, OH 43215. Please note filing company’s FEIN# in memo portion of check. Worried about potential fines and penalties? The Ohio Division of Unclaimed Funds has a Voluntary Compliance Program. Once you enter into the agreement, you report your unclaimed funds from this year as well as past reporting years and you are held harmless from litigation and those pesky fines and penalties.
