Comes with a Health Savings Account (refer to “What is a Health Savings Account?” on page 12 for more information) covered under the plan, the Annual Family Deductible is shared. However, the plan includes an embedded Annual Individual Out-of-Pocket Maximum, which means you will not pay more than this amount in a calendar year for any individual covered under the plan. For prescriptions and all covered services that are not considered routine and preventive, you pay the cost until you meet your deductible. You pay a copay when you have a visit at the doctor’s office or urgent care center for covered services that are not considered routine and preventive. You pay a percentage for prescription drugs, up to a certain amount. For all other covered services, including visits to the emergency room, you pay the cost until you meet your deductible. After you meet your deductible, you pay a percentage of the cost.
