8.4 Incident report

Definition:

An incident report is a formal recording of the facts related to an incident. The report may contain any unusual event on the Boutique premises, such as theft, burglary, accident, injury, etc. Ideally, the incident report should be completed as soon as possible following the incident, preferably on the same day of the occurrence.

Situations:

1. Financial fraud

2. Offence against a person

3. Computer misuse

4. Sexual offences

5. Stock damages

6. Theft/burglary

7. Any other qualified incidents

Incident report contents:

An incident report should include the following information: 1. Circumstances of the incident

2. Date, time and location, including shift details

3. Witness details

Important Note : Using the company format for reporting is mandatory.
