We are often asked to fill out forms providing personal information, such as our name , date of birth , address and phone number . It is important to give these details correctly.

• Discuss with students situations where they might be required to fill out a form, e.g. immigration, at the post office or bank, joining a club or library . Many people now fill out forms online, where it is particularly important to provide accurate information. • Point out the important instructions, e.g. please print . Explain any terms with which students might be unfamiliar (length of stay, family name, first name) . • In the USA dates are usually given as month / day / year . Students should look out for this when completing forms.
