Mid-America Apartments, L.P. Summary of Benefits Life and AD&D Insurance

Below is a summary of the Life and Accidental Death and Dismemberment (AD&D) coverages provided through Unum effective January 1, 2025.

Eligibility

Regular, full-time associates scheduled to work 30 hours or more per week

New Associate Waiting Period

1 st of the month on or after 90 calendar days of employment

Basic Life: Coverage is equal to 1x annual salary rounded to the next $1,000, with a minimum of $50,000 and maximum of $500,000. Premiums are paid by MAA. Basic AD&D: Coverage is equal to 1x annual salary rounded to the next $1,000, with a minimum of $50,000 and maximum of $500,000. Premiums are paid by MAA. Optional Employee Life: Coverage can be elected in increments of 1x annual salary, rounded to the next $1,000, up to the lesser of 5x annual salary or $500,000. Premiums are paid by you through bi-weekly payroll deduction. Optional Employee AD&D: Coverage can be elected in increments of 1x annual salary, rounded to the next $1,000, up to the lesser of 5x annual salary or $500,000. Premiums are paid by you through bi-weekly payroll deduction. Spouse Life: Coverage can be elected in increments of $5,000 up to $250,000. You must elect Optional Employee Life to elect Spouse Life and the amount of Spouse Life coverage cannot exceed 50% of your Basic Life and Optional Employee Life coverages combined. Premiums are paid by you through bi- weekly payroll deduction. Spouse AD&D: Coverage can be elected in increments of $5,000 up to $250,000. You must elect Optional Employee AD&D to elect Spouse AD&D and the amount of Spouse AD&D coverage cannot exceed 50% of your Basic AD&D and Optional Employee AD&D coverages combined. Premiums are paid by you through bi-weekly payroll deduction. Child Life: Coverage can be elected in increments of $5,000 up to $15,000 for one or more children up to age 26. You must elect Optional Employee Life to elect Child Life. Premiums are paid by you through bi-weekly payroll deduction. Child AD&D: Coverage can be elected in increments of $5,000 up to $15,000 for one or more children up to age 26. You must election Optional Employee AD&D to elect Child AD&D. Premiums are paid by you through bi-weekly payroll deduction.

Optional Employee Life: A statement of health is required for the additional amount elected over the guaranteed issue of $300,000. If denied, your coverage will default to the guaranteed issue of $300,000 or your current coverage amount if your current coverage amount is higher than $300,000. Spouse Life: A statement of health is required for the additional amount elected over the guaranteed issue of $50,000. If denied, your coverage will default to the guaranteed issue of $50,000 or your current coverage amount if your current coverage amount is higher than $50,000

Not Currently Enrolled:

Optional Employee Life: A statement of health is required if the amount elected is over the guaranteed issue of $300,000. If denied, your coverage will default to the guaranteed issue of $300,000. Spouse Life: A statement of health is required if the amount elected is over the guaranteed issue of $50,000. If denied, your coverage will default to the guaranteed issue of $50,000. Optional Employee Life: A statement of health is required if the amount elected is greater than the guaranteed issue of $300,000 or an increase of more than one level (1x annual salary) at each annual enrollment. Spouse Life: A statement of health is required if the amount elected is greater than the guaranteed issue of $50,000 or an increase of more than one level ($5,000) at each annual enrollment.

Evidence of Insurability (Statement of Health) Rules for New Associates and Enrollment Events After 1/1/2025

When you reach age 70, life benefits reduce to 65%. When you reach age 75, life benefits reduce to 50%.

Reduction Schedule

Waiver of Premium

Your cost may be waived if you are totally disabled for a period of time.

If you are diagnosed with a terminal illness with less than 12 months to live, you can request 50% of your life insurance benefit while you are still living.

Living Benefit

You may be able to keep all or part of your coverage(s) if you leave MAA, retire or change the number of hours you work per week (i.e., full-time to part-time).

Portability and Conversion

Life Planning Financial & Legal Resources (Included with the Basic Life coverage)

Service available to associates, spouses and beneficiaries who need help during a terminal illness or after the loss of a covered associate.

Travel assistance services are provided by Assist America, Inc. and provide 24/7 access to help with situations that may arise during travel. Services are available to you (and your spouse and dependent children up to age 26 if traveling with you) while traveling for business or pleasure more than 100 miles from home. Assist America arranges and pays for 100% of the services the company provides, but you must call Assist America first – you cannot be reimbursed for services you arrange on your own.
