Paid Parental Leave (PPL) The Company provides up to one week of Paid Parental Leave to Team Members who have been employed by the Company for 12 consecutive months and have worked at least 1250 hours in the 12 months immediately preceding the birth or adoption of a Team Member’s child. Please contact your HR Manager to initiate this leave. To document the birth, you will need to provide a birth certificate, or any other document that contains the Team Members’ name and the date of the birth. For adoptions, you will need to submit copy of the Court’s Order placing the child in the Team Member’s home. Pay for Paid Parental Leave will be calculated the same as PTO pay. You may use paid parental leave at any time within 30 days after a birth or adoption of a child. Initiate leave thr ough Unum 866 -779- 1054 www . unum . com Paid Pregnancy Disability Leave (PPDL) The Company offers Paid Pregnancy Disability Leave to female Team Members. There are certain criteria that must be met to be eligible. • Eligibility will be based on having completed 1 year of continuous employment preceding the start of the leave. • Begins on the date of the disability due to pregnancy. • Can be started up to 2 weeks pre-birth. • Maximum total duration of up to 12 weeks. After 8 weeks of paid pregnancy disability pay, medical documentation of a continuing disability will be required to receive up to an additional 4 weeks of paid pregnancy disability pay. • Continuous leave only – intermittent PPDL is not allowed. • 100% salary replacement based on actual previous 12-month earnings (same definition for STD/LTD annualized earnings). • Offsets will be applied when an active STD claim is running concurrently. (Asbury is paying 40% of the salary, while STD is paying 60%). • Cannot take PTO concurrent with PPDL. FMLA will run concurrent with PPDL. You initiate PPDL and FMLA by calling UNUM – (866) 779-1054. The Company reserves the right to modify any policy as necessary, in its sole discretion, to the extent permitted by law. Violation of any Company policies or procedures can result in disciplinary action up to and including termination of employment.
