Perinatal Program: Preparing for the unexpected
Going in for routine tests is just part of being pregnant. But what if the results reveal worrisome news and a life-threatening diagnosis for the baby? In an instant, the happiest moment of life can turn into one filled with a lot of uncertainty. Who does the family turn to for support and guidance? Community PedsCare’s Perinatal Program works closely with the entire family to develop and fulfill their unique care needs. Whether it is help with the birth plan, figuring out the next steps after delivery, guidance on coping with the diagnosis, support for siblings, or spiritual support, we are here for every step of the care journey. Our perinatal team is composed of Advanced Registered Nurse Practitioners, nurses, child life specialists, social workers, and spiritual specialists who communicate and work alongside the medical team throughout the pregnancy, delivery, and postnatal period.

Palliative Care: Living life as fully as possible
Community PedsCare’s Pediatric Palliative Care offers compassionate care for children diagnosed with a life-changing illness. Whether it’s a recent diagnosis with a serious illness or a complex, ongoing condition, palliative care works alongside existing treatments to provide the best possible quality of life at any and every stage. Community PedsCare tailors the care to what best suits the patient and family. We work to help with providing comfort and relief from symptoms, navigating difficult decision-making, ensuring that the child gets the best care possible, and connecting the family to resources and support. The care provided is an active partnership between the patient, the family, the medical team, and the pediatric palliative care team that includes: Advanced Practice Registered Nurses, nurses, social workers, child life specialists, music therapists, spiritual support, and volunteers. Together, we work to maximize the positive and meaningful experiences and minimize the physical and emotional pain for the child and the family.

Hospice Care: Making each day a little better and easier for the entire family
For children or teens whose physicians confirm a need for a higher level of care, Community PedsCare offers pediatric-specific hospice care. Caring for a child when he or she is sick is already difficult, and we know making a call to hospice isn’t easy; however, making that call does not mean giving up. Our PedsCare Hospice team delivers compassionate care in the comfort of the home while surrounded by the people and things the child loves. The smell of a hot dinner, the bark of the dog, the laughter of siblings playing, the feel of a favorite blanket, and toys all remain a part of daily life. Under the Affordable Care Act, hospices can provide concurrent pediatric care to children with life-threatening health problems who are enrolled in Medicaid or TRICARE. Parents no longer need to make the decision between getting curative treatment and hospice; they can have both. There is no treatment lost, instead, the family gains the additional supportive skill sets of the Community PedsCare Hospice Team. Our specialized team works closely with parents and the child’s medical team to ease symptoms, manage pain, decrease stress, provide emotional and spiritual support, and improve the quality of their daily activities.

Respite: Because caregivers need a break, too
We know that your child’s well-being is your priority, but don’t forget about your own health and wellness. Respite care allows families and other caregivers of Community PedsCare patients to take a break or “respite” from caregiving duties for short periods of time.

From graduation and wedding ceremonies, grocery shopping, socializing, to taking a nap or reading, respite care allows family members to enjoy simple pleasures and complete routine tasks that aren’t so simple or routine when around-the-clock care is required. During respite care, a skilled pediatric nurse tends to the child’s needs, providing peace of mind to family members that their child is in good hands.

Family Centered Care: Creating moments of Joy and Celebrating Life
Caring for a child with a life-limiting illness puts a lot of stress on a family. What most families do not know, is that at Community PedsCare we do not only provide care to the patient, but we also offer comprehensive care, education, and support to the entire family. Our team is here to listen to the family’s story, their hopes and values, and help accommodate them. The journey begins with our nurses who answer any questions and are able to guide the family through the instructions on how to administer medication and provide any necessary medical attention to their child. Our social workers step in to listen, assess, and teach. They assist in coping with the social, emotional, and financial issues that the family may be facing. From connecting to resources, and leading support groups, to facilitating events and activities, providing companionship and celebrating memories, our Community PedsCare team partners with our families by focusing on what’s meaningful to them. Support from our team continues throughout the many stages of an illness and does not stop if a loved one passes away. Our bereavement professionals are here to help families work through the grieving process and find peace through individual and group support.
