Part 4 Our Key Priorities The Quality & Safety Committee and the Board of Directors has identified three core domains that underpin our quality improvement efforts:

• Patient Safety • Clinical Effectiveness • Patient Experience

These priorities are aligned with the Care Quality Commission’s (CQC) single assessment framework five key questions, which evaluates whether services are safe, effective, caring, responsive , and well-led . We determine our annual quality priorities through a combination of staff and patient feedback, stakeholder engagement, audit findings, and service evaluations. This evidence-based approach ensures our priorities reflect both the needs of our communities and the standards expected of high-performing healthcare providers.

Our Patient Safety Statement and this Annual Quality Report set out our objectives and expectations across all areas of the quality agenda. Together, they guide our efforts to deliver meaningful improvements and ensure we continue to make a positive impact on the health and wellbeing of those we serve.
