 Primary Care Dashboard – System-Wide Opportunities We Did:

• In mid-2024, we continued to develop and enhance the Primary Care Dashboard to monitor service activity and identify emerging trends. However, the organisation we were collaborating with on this initiative was no longer able to continue their involvement, which impacted the progression of this work. In response, NPC is now exploring alternative partnerships and internal capabilities to ensure the continued development and integration of the dashboard into our wider data strategy. • We then engaged with system partners to explore integration into a wider Norfolk and Waveney dashboard, supporting collaborative planning and response to system pressures. • This has led to a deeper exploration of advanced Power BI tools to support the future design and delivery of Primary Care services. To reinforce our commitment to this strategic development, NPC has appointed a Head of Business Intelligence to lead and drive this work forward.  Embedding the Patient Safety Incident Response Framework (PSIRF) We Did: • Develop a draft Patient Safety Incident Response Policy and Patient Safety Incident Response Plan, in line with PSIRF principles. • Have planned delivery of training sessions to key staff to build awareness and preparedness for PSIRF implementation. • Conducted annual audits of significant events (SEs) and serious incidents (SIs), alongside complaints audits, to identify learning and improve processes in order to form the new approach in line with the PSIRF principles. • Initiated planning for a phased migration to PSIRF, even though it is not yet mandatory in Primary Care, demonstrating our proactive approach to patient safety. 4.2 Current Focus: Key Priorities for 2025/26 Our quality improvement priorities for 2025/26 remain aligned with the Care Quality Commission’s (CQC) five key questions: Safe, Effective, Caring, Responsive, and Well-Led .
