1. Patient Safety - Aligned with CQC domains: Safe, Caring, Responsive, Well-Led
• Ensure patients and service users are cared for in a safe environment, protected from avoidable harm. • Strengthen collaboration with health providers and the wider Integrated Care System (ICS) to enhance safety and quality across the community. • Enhance our culture of transparency and continuous improvement by regularly reviewing safety protocols and patient care practices. • Empower staff through targeted training and development initiatives, ensuring they are equipped to deliver high-quality, compassionate care while upholding the highest safety standards.
This will be achieved by:
Fully implementing PSIRF and using the NHS E ‘Being Fair’ tool into our learning from incidents processes. Making the Level One and Level 2 (role dependent) Patient Safety Syllabus Training a mandatory requirement to our workforce. Ensuring that all staff have undertaken the PSIRF e-learning module and deliver face to face education sessions to further embed into the culture. Engage with stakeholders and membership practices to encourage the implementation of the PSIRF principles.
2. Clinical Effectiveness - Aligned with CQC domains: Effective, Safe, Caring
• Improve outcomes by embedding best practices and implementing NICE guidelines across services.
• Participate in national and local clinical audits to monitor and improve care quality.
• Support staff wellbeing through structured clinical supervision and access to professional development.
This will be achieved by:
Work to increase ease and time to complete audits, whilst balancing this with continuation of service delivery.
