THE PROBLEM Cancer is astoundingly pervasive. Approximately one in two men and one in three women will face a cancer diagnosis during their lifetime. As standards of care change rapidly and disparity in provider quality pervades, sadly we know the following to be true as a result: ✓ Up to 25% of cancers are misdiagnosed or mis- staged ✓ Incorrect diagnoses lead not only to increased expenses for the employer, but also to unnecessary treatment ✓ for the employee ✓ Expensive care does not always lead to the best outcome ✓ Most utilization management occurs after the treatment plan has been determined

The coordinator will: ✓ Coordinate a second opinion with renowned national centers of excellence ✓ Promote the utilization of clinical pathways prior to treatment planning ✓ Ensure the most evidence-based and cost-effective treatment protocol is implemented quickly ✓ Help the patient navigate all aspects of care ✓ Connect the patient to the Cancer Centers of Excellence Network for complex treatment needs ✓ Facilitate access to oncology experts who work to improve care and reduce costs

Life, AD&D & Disability

• Employer paid Life and Accidental Death (AD&D) is provided at 1x Salary, up to $50,000.

• Employer paid Long Term Disability is provided up to 60% of earnings to a maximum of $7,500 (monthly) after 180-day elimination period through SS retirement age

• Voluntary Short-Term Disability provides employees option to purchase additional disability benefits at 60% of weekly earnings to a maximum of $1,500 (weekly) with a 7-day elimination period for up to 25 weeks.

• Voluntary Life Insurance provides employees the option to purchase additional life insurance to a $300,000 maximum benefit

Pre-Tax Benefits – FSA, HSA & 401k

FSA • Commuter Plans - used to pay for public transit — including train, subway, bus, parking, etc., as part of your daily commute to and from work. ➢ Transit & Parking pre-tax limit of $315/month each (totaling $630/month) ➢ Enroll through Ceridian

➢ Changes made before the 25 th of the month, will take effect the first of the following month. ➢ Changes made after the 25 th of the month, it will take effect the first of the month after next.

1. Navigate to the Ceridian Home Page 2. Select Benefits 3. The 2024 Open Enrollment will be listed on the Overview tab ❖ Be sure to complete your enrollment by December 10 th to have coverage in place on Jan 1 st
