#400 | Lessons Learned: 5 years of Providing the Full SUD Continuum of Care This session will focus on transitions of care along the ASAM continuum for substance use disorder (SUD) services. What has 5 years of data shown us? Presenters will share lessons learned about how offering the full ASAM continuum supports engagement in treatment and successful transitions back to the community following residential and hospital stays. Jennifer Holcomb is Behavioral Health Benefits Section Manager at HCPF, where she leads policy development and implementation for behavioral health services. With more than 30 years of experience, her background spans research, organizational management, and direct service to vulnerable populations. #401| Universal Adoption of Outcome Tools Over 5 years, Diversus Health has embedded longitudinal measurement-based care by integrating outcome tools throughout our clinical practice. This session reviews our phased implementation process, staff training, automation, and data-driven strategies that facilitated a rise in utilization from 3% to 70%. Attendees will gain insights on practical application of standardized outcome tool utilization in behavioral health service delivery. Lindsay Gries, PsyD has served as Director of Internship Training at Diversus Health since 2014. Lindsay’s professional passions include provision of supervision and training to Doctoral and Master’s level students, supporting the professional development of clinical leaders, and improving quality of clinical practice through implementation of measurement-based care.
