NATIONAL END OF LIFE SURVEY QUESTIONNAIRE
What is the survey about? The National End of Life Survey is a nationwide survey asking bereaved relatives about the care provided to their loved one in the last days of their life. The purpose of the survey is to learn from people’s experiences of end of life care in order to improve the services provided both to people who are dying, and to their loved ones. Why did I get this questionnaire? You got this questionnaire because you registered the death of a family member or friend between 1 September and 31 December 2022.
Can I do the questionnaire online? Yes, please go to survey.yourexperience.ie to complete the survey online.
Can I ask someone to help me fill in the questionnaire? Yes, you may wish to ask someone to help you answer some of the questions. Alternatively, you may pass the survey on to the person who knows most about the care provided to the person who has died.
