3.5 Incident reporting and investigations Our approach to incidents is that of learning and improvement. We have a well-established process for incident reporting and investigating them, which all staff are appraised of and are supported to utilise should there be anything which causes concern to them. The concerns could be actual or potential, both of which are treated in a way to instil learning and development of improvement in service delivery as a result of any investigations which may take place as a result of the reporting processes. During the period of 1 st April 2023 to 31 st March 2024, there were a combined total of 56 reported incidents which were investigated and actioned. They were in the following categories:

Accident & Incident = 8 Safeguarding = 5 Significant / Learning Event = 40 Serious Incident = 3
When compared to the period in 2022/23, where we received a total of 2 Accident & Incident reports, 28 Significant & Learning events, 1 Safeguarding incident, and 1 Serious Incident, there a notable increase in Significant and Learning events. This is attributed to the increase in clinical services which NNPC has delivered in this period. The overall demonstration of the harm grading for all reported incidents during the 2023/24 period is:
