C igna Healthcare Critical Illness Insurance Y our Cigna Healthcare SM Critical Illness insurance plan comes with a $50 W ellness Treatment, Health Screening Test or P reventive Care incentive benefit. This benefit is paid for each covered person who completes at least one wellness t reatment, health screening test or preventive care service, as specified below. This benefit is limited to one per year per c overed person. W ellness treatments • A dult immunizations

H ealth screening tests • B one marrow testing • B reast cancer blood test (CA 15-3) • B reast ultrasound • C hest x-ray • C olon cancer blood test (CEA) • C olonoscopy • F asting blood glucose test

• A nnual routine preventative dental exam • A nnual routine ophthalmological exam i ncluding refraction • C ancer screenings • C olorectal cancer screenings • G eneral health exams

• L ead poisoning screenings • O steoporosis screenings • R outine gynecological exams • R outine prostate exams • W ell child care – including visits, l abs and immunizations

• F lexible sigmoidoscopy • H emocult stool specimen • M ammography • M yeloma blood test (serum protein e lectrophoresis) • O varian cancer blood test (CA125) • P andemic infectious disease immunization • P ap smear for women over age 18 • P rostate-specific antigen (for prostate cancer) • S erum cholesterol test to determine l evels of HDL and LDL • S tress test on a bicycle or treadmill • T hermography • T riglycerides blood test

P reventive Care T he Patient Protection and Affordable Care Act (PPACA) requires preventive health services as recommended by the f ollowing expert medical and scientific bodies:

1. T he United States Preventive Services Task Force (USPSTF); 2. T he Advisory Committee on Immunization Practices (ACIP);

3. T he Health Resources and Services Administration (HRSA’s) Bright Futures Project; and 4. H RSA and the Institute of Medicine (IOM) committee on women’s clinical preventive services.
