An average of 140 older adults will be receiving home care through a consumer-directed care program during each six-month reporting period. Maintain HCBS services for older adults in Pima County as documented through agency list of services and client list in DAARS system. Clients show a high level of satisfaction as shown by surveys – at least 85% of clients will rate their service good to excellent. Hours of case management provided – 13,000/yr.

Continue provision of adult day care/adult health care, attendant care, case management, grocery shopper, home repair and renovation, housekeeping/homemakers, and personal care for those determined to meet eligibility requirements for the program.
