Collaboration focused on positive safety outcomes for mutual clients as reported by all partners – documented through meeting minutes, participant notes, or email communications. Number of meetings between PCOA and APS to discuss common concerns, mutual referrals, and clients – 24/yr. Number of older adults referred from APS to PCOA’s emergency human services program – average of 2 per week.

Maintain productive collaboration with Adult Protective Services to provide Emergency Human Services and other necessary services to older adults in need. PCOA currently has the ability to sustain up to 78 referrals per year and will continue to work with APS to build up to that from the current referral rate.
