Information and education about options and resources are a large component of every service offered. For services offered in various locations such as senior centers, the days and times are scheduled to coincide with lunch times to better accommodate the individuals going for a meal. Walk-ins are also welcome at the various locations including the PCOA main office and the Katie Dusenberry Healthy Aging Center. PCOA has developed numerous resource lists for services that are available in Pima County. These lists are distributed to clients along with support and information to ensure that the client may make an informed choice about a service that will meet his/her need. The lists are regularly updated, and new lists are created when a particular need is identified. One example is a list of transportation resources that includes fee-for-service transportation, wheelchair-accessible van rentals, volunteer ride services, subsidized rides to senior centers for meals, and airport transportation. Brief descriptions are provided for each agency or business on the list and include any eligibility requirements, parameters of the service and how to access the service. PCOA also distributes a Guide to Services that is a directory of available services in Pima County with contact information. The Guide is ordered by type of service to streamline the ability of clients to find the services they need.
8. What strategies will the agency use to coordinate with health care systems in the planning and service area to avoid duplication of services and maximum available resources?
PCOA will continue to network with and extend invitations to the health care system to be partners in advancing the work of particular initiatives including the End-of-Life Care Coalition and the Southern Arizona Falls Prevention Coalition. These Coalitions have especially sought participation from area hospitals, community health clinics and regarding the Fall Coalition, area fire departments/districts. Contracts with segments of the health care system will be continued and strengthened when possible. These contracts have included the provision of case management and evidence-based health promotion programs for health plan enrollees. PCOA will also maintain a working relationship with the Pima County Health Department, where PCOA’s President & CEO is a member of its Ethics Committee, in the facilitation of the Southern Arizona Falls Prevention Coalition, the provision of trainings for staff and volunteers hired to lead Healthy Living classes and participation in the designated Healthy Pima 2.0 Action groups.
9. What strategies will the agency implement to support consumer control and choice in your planning and service area?
Older adults participating in socialization, meals, fitness, and other offered services can choose where in the region they go for those services. They may select certain senior or neighborhood centers because of geographic location or based on the age-range, activity level, or other factors that suit them best.
