PATIENT + PT = A STRATEGY FOR SUCCESS

These newsletters travel out every month to over 5000 current or former patients. In each letter the basic message is, “How can we help you learn to help yourself with your body’s performance and comfort?” For many, comfort and performance become a growing concern as they get older or after an injury and the question becomes, “How do I avoid what looks like more difficult times ahead with pain, stiffness, weakness and reduced ability to handle my day and life?” Believe it or not, this is one of the most important questions you can ask yourself because it indicates you are thinking about the future. Many people barely think about tomorrow in terms of what it will bring versus looking long term at what life holds in the balance. Unfortunately the rude reality is that what life holds in the balance is often what we put there. By asking the “how do I avoid” question and having the willingness to look ahead to anticipate what is to come is indicative of a person who is quite responsible and proactive in their life. Many of us have learned some of the hard realities of missed opportunity when we live without putting attention into the future. Certainly we could see how this can be without good financial or insurance plans. Keeping your health can be one of those missed opportunities as well, but the truth is our health is one of those assets in life that is not often appreciated until we are not as healthy. Seeing your health as an asset is important because it invites the question we ask for all assets, “How do I protect it?” Howyou avoid something is by being able to look ahead and recognizewhere you are heading. It is important to see when it comes to our musculoskeletal health that what we ascribe to time or aging is often not the full picture of why we hurt.

Arthritis and age are often promoted as the sole reason for our pain and yes, they are two factors to consider. However, the full picture of why our performance and comfort degrade over time also has a great deal to do with how our muscle systemperforms in organizing our motion into efficient patterns of movement. Actions such as moving from sitting to standing, climbing stairs, going for a longer walk, or lifting and carrying objects require the coordinated interactions of muscles working as a team. Where does stress land whenmuscle performs inadequately? Right, on your joints! Poorly performing muscle is often the precursor to eventual joint changes and arthritis!! Realizing that muscle is changeable, that it can degrade when not properly challenged helps one see that the bigger target for recovering lost strength, motion and ability often relates to upgrading our musculoskeletal system to perform well. And the good news on this idea is muscle can improve, it can get stronger and through the right challenges providemore control to perform better. And so, people are not as stuck in their problem as they might have thought or as it is presented in the medical world. Where the rubber meets the road in life is how our bodies perform daily in our routine tasks. It can be a bit startling to realize we actually have over 600 muscles steering over 200 bones in our body. There is nothing about life, gravity, work and imbalanced postures and injuries that lend toward promoting the efficient steering of bones by muscle over time.

It is essential a person realizes they need an exercise strategy, not unlike a financial strategy for retirement, to really answer the question, “How do I avoid the typical body breakdown tendencies as I get older?” I truly believe the body is built well but it does have key requirements it must maintain to hold its efficiency in movement. Muscle is at the heart of this strategy and training it is the science and art of physical therapy. Physical therapists aremovement specialist and organizing yourmuscle system for performance and control is how life can become much more comfortable, enjoyable and interesting. Yes, you will have to work at it and devote your own time to make good things happen but is this not true of most things in life where your effort yielded a positive outcome and a great deal of satisfaction in accomplishing a goal? There is an old saying, “Happiness is the overcoming of not unknowable obstacles toward a known goal.” As PTs, we identify the obstacles and provide the knowhow to overcome and you reap the joy of reaching a goal, perhaps one you had given up on . We call this active care as opposed to passive care (medication, rest, massage, adjustments, injections, surgery, etc.). Ultimately, we need an active care strategy to navigate the pitfalls of time and aging. Passive care has a place in one’s recovery after an injury but it should not be the only tool in our toolbox.

Perhaps the best tools in life relate to knowledge which can be used over and over as needed, kind of like the gift that keeps on giving. Herein is described a true result, one that got to the core of the problem and rectified it. Muscle is your ally in the battle with gravity. Muscle works best as a team effort where good muscle organization promotes efficiency in movement. This is what your body hadwhen younger and it declinesmore due to imbalances inmuscle strength and tightnessmixing together to disorganize movement and function. You cannot change osteoarthritis or your age and when these are given as the only reason why you hurt then it is a bit disheartening to feel there is not much you can do but medicate and curtail your activity. Seeing muscle as something that is upgradable renews a sense of hope in your future. Asking the question, “How do I avoid what looks likemore difficult times ahead with pain, stiffness, weakness and reduced ability to handle my day and life,” can be the beginning to a better future if you are willing to take action. Today is a newday, a newopportunity. Like a good financial plan, it will pay you back over and over. Isn’t that the definition of an investment?

Seasonal Recipe

Ingredients • 1 1/3 cups all-purpose flour • 1/2 tsp baking powder • 1/2 cup chocolate hazelnut spread • 1/2 cup sugar • 1/2 cup brown sugar • 1 egg • 1 tsp vanilla extract • 3/4 cup chopped toasted hazelnuts C H O C O L AT E H A Z E L NU T B I S C O T T I Instructions Preheat oven to 375˚. Combine flour, baking powder, baking soda, and salt in a bowl. Set aside. In another bowl, combine butter, chocolate hazelnut spread, sugar, and brown sugar. Using an electric mixer, cream ingredients together, about 4 min. Add egg and vanilla and beat until smooth, about 1 min. Using a wooden spoon or rubber spatula, stir in flour mixture until mixed. Add hazelnuts and stir until mixed. Spoon out the cookie dough onto a cookie sheet, spacing the mounds about 4” apart. Use the tines of a fork to flatten the cookie dough. Bake until lightly golden around the edges, about 10-12 min. Use a metal spatula to transfer the cookies to a wire rack and let cool. • 1/2 tsp baking soda • 1/4 tsp kosher salt • 1/2 cup butter, softened

TRUNK FLEXION & EXTENSION

Lay on your back, bringing your knees to your chest. Grab your toes and hold for 10 seconds. Repeat as needed.

Sit yourself tall on the edge of a chair with your feet flat on the ground. Place your hands on your thighs. Inhale while leaning you trunk towards your thighs, lifting your chest. Then exhale while arching your back into a chair, rounding your shoulders as shown. Repeat 6-10 times.
