• Monthly touch points with your Personal Wellness and Care Coordinator • Annual assessments that proactively identitfy potential issues • A plan that will support your wellness and successful aging goals and address potential issues, as appropriate
• Support from your Personal Wellness and Care Coordinator wherever you are, including on vacation • Coordination of transitions between residential living and higher levels of care as appropriate
